杜氏肌营养不良症(DMD)是一种全身肌肉呈渐进性损伤和运动功能减退的致死性X 连锁隐性遗传病。
患儿多于3~4岁出现步态异常,10~12岁逐渐丧失行走能力,最终在20~30岁死于心肺功能衰竭。
男婴发病率为1/3500,我国每年有400~500例DMD 男婴出生,累计患者7万~8万人,是世界上该病患者人数最多的国家之一。
该病发病原因是位于Xp21.2-p21.1的编码抗肌萎缩蛋白(Dystrophin)的DMD 基因突变,导致Dystrophin 蛋白在肌细胞膜上缺失,患者肌细胞变性、坏死继而脂肪结缔组织增生。
DMD 基因突变类型有多种,大片段缺失占60%~65%,大片段重复占5%~10%,点突变、小片段缺失和重复占25%~30%。
虽然目前尚无非常有效的治疗方法,但随着对DMD 的发病机制和病理变化过程愈渐深入的认识,对其治疗的研究与相关临床试验有了很多新的突破。
本文对DMD 近年来相关研究进展综述如下:1治疗方法1.1对症治疗药物Dystrophin 蛋白缺失会导致一系列病理变化,包括肌肉萎缩、炎症、钙过载、氧化压力、组织缺血等。
针对这些病理变化过程的相关药物治疗可以使所有患者获益。
1.1.1激素激素是目前认为能够减慢DMD 患者肌力和运动功能衰退的唯一药物,长期每日服用糖皮质激素能明显延缓患者病情进展。
它的作用机制主要是通过抑制NF-κB 通路抑制炎症,激活钙调磷酸酶NF-AT 通路抑制肌坏死。
目前强的松和强的松龙是最常用的激素治疗药物,美国FDA 批准的地夫可特也可以用于DMD,且副作用更小[1]。
近年来正在研发温和激素Vamorolone(VBP15),相对于传统激素,能以更小的副作用获得同等疗效[2]。
