【摘要】目的探讨宫腔镜下子宫内膜息肉切除术后放置左炔诺孕酮宫内缓释系统(LNG-IUS)对预防复发的效果.方法选取2014年2月至2016年2月收治的子宫内膜息内患者87例,采用随机数字表法分为两组,研究组43例在宫腔镜下子宫内膜切除术后及时在宫内放置左炔诺孕酮宫内缓释系统,对照组44例手术后不做任何处理.对所有患者进行12个月随访,记录术后3、6、12个月患者的子宫内膜厚度、月经量变化及息肉复发情况.结果两组患者术前子宫内膜厚度及月经量差异无统计学意义(P＞0.05),术后3、6、12个月两组的子宫内膜厚度及月经量均明显减小(P＜0.05);研究组术后各时间点的子宫内膜厚度明显变薄(P＜0.05),优于对照组;研究组月经量随时间延长逐渐减少,各时间点与对照组比较差异有统计学意义(P＜0.05).研究组术后息肉复发率为4.65％,明显低于对照组的25.00％ (P＜0.05).结论宫腔镜下子宫内膜息肉切除术后放置LNC-IUS,能够抑制内膜增生,减少月经量,有效预防息肉复发.%Objective To investigate the effect of placing levonorgestrel slow-releasing intrauterine system(LNG-IUS) in preventing recurrence after endometrial polyp resection under hysteroscopy.Methods Eighty-seven patients with endometrial polyps in our hospital from February 2014 to February 2016 were selected and divided into two groups according to the random number table method.Forty-three cases in the study group were
timely placed with LNG-IUS after hysteroscopic endometrial
resection,while 44 cases in the control group did not receive any treatment after the surgery.All cases were followed up for 12 months.The endometrium thickness,menstruation amount change and polyps recurrence were recorded at postoperative 3,6 and 12 months.Results The endometrial thickness and menstrual volume had no statistical difference between the two groups(P＞0.05).The endometrial thickness and menstrual volume of the two groups were significantly decreased at 3,6 and 12 months after operation(P＜0.05).The endometrial thickness at each time point in the study group was significantly thinner than that in the control group(P＜0.05),which was superior to the control group;the menstruation volume in the study group was gradually decreased with the time extending,which at various time points had statistical difference compared with the control group(P＜0.05).The recurrence rate of endometrial polyps in the study group was 4.65％,which was significantly lower than 25.00％ in the control group(P＜0.05).Conclusion The placement of LNG-IUS after endometrial polyp resection can inhibit endometrial hyperplasia,reduces the menstruation amount and effectively prevent the recurrence of endometrial polyps.
