摘 要:目的 探讨 HBV(乙型肝炎病毒)感染在辅助生殖技术中对胚胎发育和妊娠结局的影响。方法 回顾性 分析 2010 年 1 月至 2012 年 12 月在南京市妇幼保健院生殖医学中心首次行 IVF-ET( 体外受精 - 胚胎移植 ) 治疗 的 278 对感染 HBV 患者夫妇的临床资料,患者均为输卵管因素、男方因素或不明原因导致的不孕。根据 HBsAg(乙 肝表面抗原)阳性情况进行分组,分为:单独男方阳性组,146 个周期;单独女方阳性组,98 个周期;夫妇双方 阳性组,34 个周期。对照组为同一时期在南京市妇幼保健院生殖医学中心首次行 IVF-ET 治疗的未感染 HBV 的 不孕夫妇,350 个周期。比较各组的基本临床资料、超促排卵结果、胚胎着床率和临床妊娠率是否存在统计学差 异。结果 单独男方阳性组、单独女方阳性组、夫妇双方阳性组和对照组的基本临床资料、超促排卵结果差异无 统计学意义(P > 0.05)。单独男方阳性组和单独女方阳性组的胚胎着床率和临床妊娠率与对照组相比差异无统 计学意义(P > 0.05)。夫妇双方阳性组的胚胎种植率、临床妊娠率与对照组相比差异有统计学意义(P < 0.05)。 结论 只有男方或者女方一人为 HBsAg 阳性的夫妇行 IVF-ET 对胚胎发育和妊娠结局的影响不显著。夫妻双方均 为 HBsAg 阳性行 IVF-ET 对胚胎发育影响不显著,但其不良妊娠结局率显著高于正常对照组。 关 键 词 : 乙型肝炎病毒;体外受精;胚胎移植;辅助生殖技术

ABSTRACT: Objective To investigate the influence of hepatitis B virus infection on embryo development and outcomes in assisted reproduction treatment. Methods The clinical data of 278 couples with hepatitis B virus infection whichwas underwent IVF-ET treatment for the first time in the Assisted Reproductive Unit of Nanjing Maternity and Child Health Care Hospitalform January 2010 to December 2012were retrospectively analyzed. All of them were caused by fallopian tube factor or male factor or other factors. Based on the positive situation of the HBsAg,the patients were divided into male with hepatitis B virus infection group (n=146), female with hepatitis B virus infection group(n = 98) , both with hepatitis B virus infection group(n=34)and control group(without hepatitis B virus infection, n = 350). The general condition of the clinical data, outcome of ovarian stimulation, implantation rate and clinical pregnancy rate were compared. Results The difference of general condition of the clinical data and the outcome of ovarian stimulation in these four groups were not statistically significant (P> 0.05). There were no statistically significant differences in the implantationrate and clinical pregnancy rate among the male positive group, the female positive group and the control group(P> 0.05).The difference of implantation rate and clinical pregnancy rate in group C and group D were statistically significant (P< 0.05). Conclusion The hepatitis B virus infection only in the half part of the couple which was underwent IVE-ET treatment has no significant effect on embryo development and outcomes of the in vitro fertilization and embryo transfer treatment. The hepatitis B virus infection in both two of the couple has no significant effect on embryo development but the adverse pregnancy outcome rate was significantly higher than the control group.
