平喘方治疗小儿支气管哮喘不同证型发作期临床研究

【摘要】目的平喘方治疗急性发作期的哮喘患儿,临床疗效显著,现拟进一步评价该方治疗小儿哮喘急性发作期寒哮、热哮、外寒里热型哮喘的临床疗效,以验证其痰瘀互结学说,丰富中医药治疗小儿哮喘的理论和治法.方法将200例哮喘发作期患儿随机分为对照组和观察组,每组100例.对照组寒性哮喘选小青龙汤加减,热性哮喘选麻杏石甘汤加减,外寒里热选大青龙汤加减;观察组用平喘方加味治疗,寒性哮喘选平喘方合小青龙汤加减,热性哮喘选平喘方合麻杏石甘汤加减,外寒里热型选平喘方合大青龙汤加减,疗程均为1周,比较治疗前后症状、体征和肺功能等方面的差异.结果最终共180例患儿完成本次研究,观察组和对照组各90例.治疗后2组中医证候积分改善率(%)比较,差异有统计学意义(P＜0.05);治疗后2组FEV1数值、FEV1/FVC预计值、PEF预计值比较,差异有统计学意义(P＜0.05);治疗后寒性哮喘组与热性哮喘、外寒里热组比较哮鸣音消失时间比较,差异有统计学意义(P＜0.05).结论平喘方治疗哮喘急性期有较好疗效,尤其对哮喘咳痰、肺功能和哮鸣音的改善方面具有优势,且用药安全.%Objective Pingchuan decoction is effective in treating asthmatic children with acute attack, to evaluate the effectiveness and safety of Pingchuan decoction in treating acute bronchial asthma, verify the theory of phlegm and blood stasis and further enrich the theory and treatment of children asthma. Methods 200 cases of asthma children were randomly divided into control group and experimental group , with
100 cases in each group. The control group cold asthma choose Xiaoqinglong decoction, heat asthma selection of Maxing Shigan decoction, cold in the heat asthma of Daqinglong decoction; modified by Pingchuan decoction treatment group, cold asthma chooses Pingchuan decoction combined Xiaoqinglong decoction, heat asthma and selection of Pingchuan decoction of Maxing Shigan decoction. Cold in the heat type selection of Pingchuan decoction and Daqinglong decoction, the course of treatment was 1 weeks. The improvement rate of TCM syndrome score, changes of lung function before and after the disappearance time of asthma between the two groups were observed and compared. Results A total of 180 children completed the study, including 90 in the observation group and 90 in the control group.The two groups after treatment of TCM syndrome integral improvement rate (%), it has a statistically significant difference (P＜ 0.05); The two groups after treatment of FEV1/FVC, PEF expected value comparison, date has a statistically significant difference (P ＜0.05); There was a significant difference in the disappearance time of asthma between the treatment group and the group of hot asthma and external cold heat (P＜ 0.05). Conclusion There are good curative effect Pingchuan decoction in the treatment of acute phase of asthma, especially has the advantage of sound and improve the pulmonary function and asthma sputum, and drug is safe.
