(山东省临的 探讨单核细胞趋化蛋白 -1(MCP-1) 和超敏 C 反应蛋白(hs-CRP)等细胞因子水平的变化在 2 型糖尿病视网膜病变 (DR) 发病机理中的作用。 方法 选择 2 型糖尿病合并白内障并已行白内障超声乳化手术或超声乳化联合玻璃体切割术患者 164 例 , 根据
眼底情况分为无视网膜病变组(NDR)55 例、背景期 DR 组(BDR)53 例和增殖期 DR 组(PDR)56 例 , 并收集同期的非糖尿病白 内障手术患者作为对照组,共计 50 例。所有患者于术前采集血液和术中抽取未稀释的房水 , 分别采用 ELISA 法检测血液和房水中 MCP-1 和 hs-CRP 的浓度并进行组间比较。结果 NDR 组、BDR 组、PDR 组血液及房水中 MCP-1 和 hs-CRP 的浓度明显高于对照组 (P<0.01),且与病情的进程呈正相关。MCP-1 和 hs-CRP 浓度组间差异显著(P<0.05)。结论 糖尿病患者血液及房水中 MCP-1 和 hs-CRP 与 DR 的严重程度密切相关 ,MCP-1 和 hs-CRP 在 DR 的损伤机制中有协同作用,对 2 型糖尿病视网膜病变患者的免疫机制和 治疗有指导意义。

diabetes mellitus complicated with cataract were selected and underwent phacoemulsification or phacoemulsification combined with vitrectomy,according to the fundus condition, 55 cases were divided into retinal lesion group (NDR), 53 cases of background phase DR group (BDR) and 56 cases of proliferative phase DR group (PDR). And collect non diabetic cataract surgery patients in the same period, namely the control group of 50 cases. All patients collected blood and not diluted aqueous humor before operation, the concentrations of hs-CRP and MCP-1 in blood and aqueous humor were detected by ELISA method and compared between groups. Results In NDR group, BDR group, PDR group, MCP-1 and hs-CRP concentrations in blood and aqueous humor were significantly higher than those in control group (P<0.01), and were positively correlated with the progression of the disease. MCP-1 and hs-CRP concentrations were significantly different between groups (P<0.05). Conclusion MCP-1 and hs-CRP were closely related to the severity of DR in the blood and aqueous humor of the patients with diabetes mellitus, MCP-1 and hs-CRP have a synergistic effect on the DR damage mechanism, it has guiding significance for the immune mechanism and guiding treatment of patients with type 2 diabetic retinopathy.
