行医笔记：乳房外Paget病的多种面孔
Paget 病又称湿疹样癌，是一种临床表现为湿疹样皮损的特殊类型皮肤肿瘤，组织病理以表皮内有大而淡染的异型细胞（Paget 细胞）为特点。
分为乳房 Paget 病和乳房外 Paget 病。
乳房外 Paget 病临床相对少见，容易误诊和漏诊，现一起看看它的多种面孔吧。
1. 口腔的乳房外 Paget 病

图8. 左侧腹股沟浸润性红斑，潮红浸润，表面糜烂，较多白色颗粒状肉芽组织（图源：PMID: 29991846）乳房外Paget 病多见于女性，好发于女阴，其次为阴囊、会阴及肛周，亦见于阴部以外的其他大汗腺部位。
皮损多呈界限清楚的红色斑片或斑块，表面呈湿疹样，伴糜烂、渗出或结痂，常有痛痒感，易被误诊为湿疹。
中老年人单侧乳房或大汗腺分布区发生慢性湿疹样斑片，按湿疹治疗无效时，应考虑到本病可能。
同时，口腔部位的病例报告提示乳房外Paget 病不仅仅局限于大汗腺区，更易误诊。
