【摘要】目的探讨剖宫产切口妊娠超声诊断声像图特点及介入治疗的临床效果.方法随机选取本院2013年9月至2015年4月收治的剖宫产切口妊娠患者90例,对所有患者进行阴道超声诊断,分析其声像图特征,并根据诊断结果对其中45例胚囊型患者实施甲氨蝶呤联合超声引导下孕囊抽吸术,另45例混合包块型患者仅实施甲氨蝶呤治疗,对两组临床治疗效果进行回顾性分析.结果观察组经治疗其胚囊大小[(12.5±2.7)mm]、血HCG水平[(112.9±35.7) nmol/L]均明显优于对照组[(17.2±3.4)mm、(141.2±51.5) nmol/L],差异具有统计学意义(P＜0.05).结论妊娠超声检查能够为剖宫产切口妊娠早期诊断提供可靠的证据,其中超声引导下的介入治疗能够有效抑制胚胎发育,是一种有效的治疗方法.%Objective To investigate the ultrasonographic characteristics of cesarean scar pregnancy and the clinical effect of the interventional therapy.Methods 90 patients with cesarean scar pregnancy admitted into our hospital from September,2013 to April,2015 were randomly selected.All the patients were diagnosed by transvaginal ultrasound.The sonographic characteristics were analyzed.According to the diagnosis results,45 cases of embryo sac type were given methotrexate and ultrasound-guided pregnant sac aspiration and the other 45 cases of mixed masses type were only given methotrexate.The clinical therapeutic effects of these two groups were
retrospectively analyzed.Results After the treatment,the sizes of embryo sac and blood HCG levels were significantly better in the observation group than in the control group [(12.5 + 2.7) mm vs.17.2 + 3.4) mm and (112.9 + 35.7) nmol/L vs.(141.2 + 51.5) nmol/L],with statistical differences (P＜0.05).Conclusions Ultrasonic pregnancy examination can provide reliable evidence for the early diagnosis of cesarean scar
pregnancy.Ultrasound-guided interventional therapy can effectively inhibit the development of embryo,which is an effective treatment method.
