患者于2天前和祖父母一道至武汉过暑假, 发作喘息,自行吸入喘康速气雾剂后症状有所缓解。
既往史:出生于武汉,自3岁起有反复发作的哮喘史,尤以春季发作次数较多,于10岁时随其父母迁往深圳居住后发作逐渐减少,仅2年几乎未再发作。
对酒精过敏。
体格检查:体温36℃,脉搏110次/分, 呼吸28次/分, 血压110/ 70mmHg。
神志清楚,自动体位,汗多。
肺部听诊双肺可闻及哮鸣音,呼气时间延长。
心率110次/分, 律齐,未闻及杂音。
腹部未见异常体征,四肢及脊柱正常。
实验室检查:支气管扩张试验示吸入支气管扩张剂(喘乐宁气雾剂)后第一秒用力呼气量(FEV1)较吸入前增加20%。
心电图示窦性心动过速。

(1分)3.鉴别诊断:1)心源性哮喘急性左心衰时,可发生呼吸困难,并可闻及哮鸣音,但心源性哮喘多有高血压、冠心病、风心病二尖瓣狭窄等病史和体征,该患者年幼,无心脏病史,显然不符合心源性哮喘。
(1分)2)喘息性支气管炎多见于中老年人,有慢性咳嗽史,有肺气肿体征,两肺常可闻及湿罗音。
该患者也不符合喘息性支气管炎的诊断。

患者近半月来无明显诱因下觉右腰部胀痛不适,伴尿频、尿急、尿痛,全程肉眼血尿,尿色暗红,无凝血块,无排尿困难,无发热、腹胀、腹痛、恶心、呕吐等不适,于当地医院行输液、抗炎治疗后症状稍缓解(具体用药不详)。
患者既往于1996年及1998年两次因―泌尿系结石‖在当地医院行手术治疗(具体术式不详)。
入院时查体:T 36.5℃R 20bpm P 80bpm BP 120/80mmHg,神清,检查合作,全身皮肤粘膜巩膜无黄染,浅表淋巴结无肿大,心肺听诊无异常,腹平软,右腰部可见一长约14cm手术瘢痕,肝脾肋下未触及,Murphy征阴性,右下腹压痛,无反跳痛,双肾区叩击痛阳性,右输尿管走行区压痛,无反跳痛,双下肢无浮肿。
辅助检查:入院后血常规、大便常规、肝肾功能、出凝血时间正常,小便常规:―BLD 3+ LEU3+‖,腹部B超提示:肝、胆、胰未见异常。
右肾内多个强回声光团,后伴声影,最大为 2.0cm×1.0cm,集合系统分离3.4cm,左肾内一个强回声光团,后伴声影,大小为0.3cm×0.3cm,右输尿管上段可见一强回声光团,后伴声影,大小为1.5cm×0.8cm。

卧位腹平片示肠管充气扩张,肠间隙增宽。
B超:肝回声均匀,未发现异常病灶,胆囊7×3×2cm大小,壁厚0.4cm,内有多发强光团,回声后有声影,胆总管直径0.9cm,胰腺形态失常,明显肿大,尤其以胰头、胰体明显,胰周多量液性暗区,胰管增粗。

入院当天顿服米索前列醇3片诱发并加强宫缩,稍后开始下腹阵痛,能耐受,同时出现阴道出血,多于月经量。
注射止血药不能减少出血,量越来越大。
孕5产1,人流3次。

即时膀胱充盈后床边B超示胎盘大部位于子宫后壁,其下缘绕过先露覆盖宫颈内口止于子宫前壁,胎盘最厚处<3cm。

指妊娠20周后或分娩期,正常位置的胎盘在胎儿娩出前,部分或全部从子宫壁剥离。
(2)先兆子宫破裂。
引产过强的宫缩如遇头盆不称,可引起子宫先兆破裂,此时可见病理缩复环。
4.处理:(6分)立即行剖腹取胎术,不仅能迅速终止妊娠,防止进一步出血,而且可在直视下处理如胎盘粘连、植入等引起的产后出血。
若家属同意,还可行双侧输卵管结扎术。
该患者虽为经产妇,胎儿不大,若考虑继续从阴道排出胎儿,因是中央型前置胎盘,所冒风险太大,同时不排除患者因产后出血不能控制,仍有剖腹止血或切除子宫可能。
六、病案分析(20分)男,8个月。
发热、腹泻3天。
患儿于3天前开始发热,体温37.5~38.5℃,下午及夜间为著。
自服退热药物体温可暂时下降,几小时后又升高。
发热后出现腹泻,大便为黄色蛋花汤样,开始每日7~8次,今天大便10余次,每次量较多,无粘液脓血。
