解郁丸治疗抑郁症的随机对照试验的系统评价

【摘要】目的系统评价解郁丸治疗抑郁症的临床疗效及安全性.方法检索电子检索数据库中国学术期刊全文数据库、重庆维普中文科技期刊全文数据库、中国生物医学文献数据(CBM)、万方数据库、美国国立医学图书馆生物医学信息数据库、Cochrane图书馆临床对照试验注册资料库中解郁丸治疗抑郁症的随机对照试验研究文献,检索日期均为建库至2016年8月.对文献进行筛选、资料提取,并进行方法质量学评价,对数据进行分析综合,结局效应指标用相对危险度(RR)或均数差(MD)表示,均以95%为置信区间.结果共纳入19篇研究文献共1 698名受试者,纳入研究的方法学质量普遍偏低.Meta分析结果显示,解郁丸联用抗抑郁西药的临床有效率(RR=1.16,95%CI 1.10~1.24;P<0.000 01)及汉密尔顿抑郁量表评分(MD=-

3.84~-1.43;P<0.000.1)优于单用抗抑郁西药治疗,而单用解郁丸治疗与抗抑郁西药疗效相当;解郁丸单用(RR=0.07,95%CI 0.04~0.13;P<0.000 01)或解郁丸联用抗抑郁西药(RR=0.53,95%CI 0.40~0.70;P<0.000 01)的不良反应发生率明显低于单纯抗抑郁西药治疗.结论解郁丸治疗抑郁症具有一定的临床疗效及安全性,然而由于所纳入的研究方法学质量普遍偏低,尚需要开展更多的大样本、高质量的随机对照试验进一步验证.%Objective To systematically evaluate the clinical efficacy and safety of traditional Chinese medicine Jieyu pill in the treatment of depression.Methods The published data of randomized controlled trials of Jieyu pill in the treatment of depression were searched
from CNKI,VIP,CBM,WanFang,Pubmed and the clinical trial registration database of Cochrane Library from the establishment of the database to August 2016.After screening and extracting the data,the methodological quality was assessed and the materials involved were analyzed.The outcome measure index was relative risk or difference of mean value(95% confidence interval).Results A total of 19 RCTs were included in the study,and the methodological quality of the included studies was generally low.Meta-analysis results suggested that as compared with antidepressants,Jieyu pill combined with antidepressants had significant advantages in clinical efficiency(RR=1.16,95%CI 1.10-1.24;P<0.000 01),HAMD scores(MD=-2.64,95%CI -3.84--1.43;P<0.000.1),whereas there was no significant difference in clinical efficiency and HAMD scores between Jieyu pill and antidepressant.Meta-analysis results suggested that the incidence of adverse reactions of Jieyu pill(RR=0.07,95%CI 0.04-
0.13;P<0.000.01) and Jieyu pill combined with
antidepressants(RR=0.53,95%CI 0.40-0.70;P<0.000.01) were lower than that of antidepressants alone.Conclusion The present evidence suggests that Jieyu pill is effective and relatively safe for patients with depression.However,because the methodological quality of the included studies is generally low,more large-scale RCTs with high quality are still needed for further verification.
