【摘要】目的探讨腹腔镜下保留盆腔神经的宫颈癌根治术的临床疗效及对患者生活质量的影响.方法收集宫颈癌患者62例作为研究对象,按照手术方法不同分组,观察组32例患者行腹腔镜下保留盆腔神经的子宫切除术(LN-SRH),对照组30例患者于腹腔镜下行子宫切除术(LRH).对两组患者术中指标、术后恢复情况、并发症以及生活质量进行考察与比较.结果观察组手术时间明显长于对照组(P＜0.05),而两组术中出血量、淋巴结清扫数目、阴道及韧带切除长度比较无显著差异(P＞0.05).观察组住院时间、肛门排气时间、肛门排便时间及拔除导尿管时间,术后7d及14 d时残余尿量均明显少于对照组(P＜0.05).观察组并发症发生率为12.50％,明显低于对照组的43.33％,差异有统计学意义(P＜0.05).两组术后6个月生活质量各条目及总分均显著高于术前,观察组得分明显高于对照组(P＜0.05).结论 LNSRH治疗早期宫颈癌安全可行,利于患者生活质量恢复,值得临床推广.%Objective To explore the clinical efficacy of laparoscopic pelvic nerve preservation radical hysterectomy for cervical cancer and influence on life quality.Methods 62 patients with cervical cancer were divided into the observation group (underwent laparoscopic pelvic nerve preservation radical hysterectomy,32 cases) and the control group (underwent laparoscopic hysterectomy,30 cases) according to different operation style.The indexes during
operation,postoperative recovery,complications and life quality of 2 groups were observed and compared.Results The operation time of the observation group was obviously higher than the control group (P ＜
0.05),but the blood loss,number of lymph node,length of vagina and ligament resection of the 2 groups had no differences (P ＞ 0.05).The hospital stays,the time of exsufflation,bowel movement and catheter removing time,residual urine volume 7 d and 14 d after operation of the observation group were all less than the control group (P ＜0.05).The complication rate of the observation group was 12.50％,which was obviously lower than 43.33％ of the control group (P ＜ 0.05).The life quality 6 months after operation were higher than before operation,and the observation group was much higher than control group (P ＜
0.05).Conclusion Laparoscopic pelvic nerve preservation radical hysterectomy is effective and safe for early cervical cancer,it is good at recovery of life quality and worthy of clinical applications.
