雷公藤及其制剂相关肝损害国内文献分析

【摘要】Objective: To investigate the clinical features and causes of liver damage related to triptolide and its preparations and provide the preventive measures. Methods: Search terms included "triptolide" in keywords. A search of Chinese journal fulltext data base (CNKI data base) and Chinese science and technology periodical database (VIP information) was done and the case reports of triptolide and its preparations-related liver damage published in domestic literatures from 1990 to 2010 were collected. The baseline characteristics of the patients, the situation of drug use, the clinical features, prognosis, and outcome of the liver damage were analyzed. Results: The total of 185 liver damage cases related to triptolide and its preparations were collected. The damage mostly occurred in the 0 -16 year-old group. After drug administration, the onset time of liver damage was as follows: the shortest was 7 days, the longest was 6 months. The majority damage cases occurred in 1 - 4 weeks. The main clinical manifestations were jaundice and abnormal liver function. After drug withdrawal, most patients were cured with receiving live-protective treatment. Of the 185 patients, 9 were cured, 123 got improved and 3 were of no effect. Conclusion: Triptolide and its preparations may induce liver
damage, which should be paid more attention to in clinical application.%目的:调查雷公藤及其制剂相关肝损害的临床特征和发生原因,以提供预防措施.方法:以"雷公藤"为索中国期刊全文数据库及维普中文科技期刊文献数据库,收集1990-2010年国内文献报道的与雷公藤及其制剂相关的药物性肝损害病例,对患者的一般情况、用药情况、肝损害的临床表现、预后和转归进行归纳分析.结果:雷公藤及其制剂相关的肝损害共185例,多发生于0～16岁年龄组,用药后肝损害发生时间最短7 d,最长＞6个月,多数发生于用药后的1个月内,临床主要表现为黄疸和肝功能异常.经过治疗,123例患者痊愈,9例患者好转,3例无效,其余不详,没有死亡病例.有3例患者因服用雷公藤制剂导致肝损害,再次给药后又发生肝损害,其中2例为雷公藤多苷片,1例为雷公藤片.结论:雷公藤及其制剂可引起肝损害,临床用药过程中应注意.【总页数】4页(P169-172)
