弯针半月神经节射频治疗三叉神经痛的有效性与安全性研究

摘 要 目的:研究利用弯针技术射频热凝毁损 (radiofrequency thermocoagulation, RFTC) 半月神经节 治疗三叉神经痛的临床疗效以及安全性。方法:选取 42 名三叉神经痛病人,按数字随机表法分成 2 组, 对照组 A (n = 20):采用直针穿刺技术进行半月神经节射频热凝毁损治疗;实验组 B (n = 22):采用弯 针穿刺技术进行半月神经节射频热凝毁损治疗。观察病人手术前及手术 1 d、3 d、7 d、1 m、3 m、6 m 后的疼痛程度、生活质量、并发症,并对术中的操作情况及病人受到的电离辐射量进行观察对比。结 果:两组病人术后同术前的基线相比,疼痛评分显著降低 (P < 0.001),生活质量评分显著提高 (P < 0.01), 组间比较差异无统计学意义;曝光时间与剂量面积乘积弯针组优于直针组,差异具有统计学意 义 (P < 0.05);两组病人均未发生严重的并发症。结论:弯针技术半月神经节射频热凝治疗三叉神经 痛效果明显,安全性高;弯针技术在减少病人电离辐射方面优于直针。 关键词 三叉神经痛;弯针;射频热凝
THE EFFICACY AND SAFETY OF CURVED NEEDLE TECHNOLOGY IN SEMILUNAR GANGLION RADIOFREQUENCY THERMOCOAGULATION FOR TREATING TRIGEMINAL NEURALGIA WURI Le-Ge1, ZHAI Zhi-Chao1, SHAN Zhi-Jing1, LIU Jin-Feng2 △ (Department of Anesthesiology, Tongliao City Hospital, Tongliao 028000, China; Department of Pain management, The Second Affiliated Hospital of Harbin Medical University, Harbin 150000, Chinlore the clinical efficacy and safety of curved needle technology for radiofrequency thermocoagulation on semilunar ganglion in the treatment of trigeminal neuralgia. Methods: Forty-two patients diagnosed with trigeminal neuralgia were randomly divided into two groups, the control group A (n = 20): the straight needle technique for radiofrequency thermocoagulation; the experimental group B (n = 22): the curved needle technique for radiofrequency thermocoagulation. Pain intensity, quality of life, complications were observed before the operation, and at 1 day, 3 days, 7 days, 1 month, 3 months, 6 months after the operation, and the operation conditions, the dose of ionizing radiation were compared. Results: Compared with the preoperative baseline, pain scores of the patients in the two groups were decreased significantly (P < 0.001), quality of life significantly increased (P < 0.01), there was no significant difference between two groups. Exposure time and dose area of the curved needle group were superior to that of the straight needle group and there was significant difference between two groups(P < 0.05). There was no serious complications in both groups. Conclusion: Radiofrequency thermocoagulation with curved needle technology on semilunar ganglion in the treatment of trigeminal neuralgia was effective and safe, and curved needle technology is superior to straight needle in terms of reducing the mount of ionizing radiation.
