【摘要】目的探讨血浆脂联素(plasma adiponectin,APN)和B型钠尿肽(B-type natriuretic peptide,BNP)检测对慢性充血性心力衰竭(congestive heart failure,CHF)患者心功能评价的作用及临床价值.方法采用双抗体夹心ELISA法检测100例CHF患者治疗前后和30例非CHF(对照组)患者APN和BNP水平,分析APN、BNP水平与心功能分级的关系.结果心功能Ⅱ～Ⅳ患者APN、BNP水平显著上升,与对照组比较差异有统计学意义(P＜0.05);APN、BNP水平随着心功能分级增高而逐渐上升,各级间比较差异有统计学意义(P＜0.05);治疗后心功能Ⅱ～Ⅳ级组患者APN、BNP水平显著下降,与治疗前比较差异有统计学意义(P＜0.05);CHF 患者血浆APN、BNP水平与CHF患者心功能级别呈线性相关性 (r=0.887、
0.984,P＜0.01).结论 CHF患者血浆APN、BNP水平与心力衰竭严重程度密切相关,APN、BNP水平变化可做为评价CHF治疗效果的临床指标.%Objective To investigate the change and clinical significance of plasma adiponectin (APN)and B-type natriuretic peptide(BNP)for evaluating the cardiac function in patients with chronic congestive heart failure(CHF). Methods The plasma APN and BNP levels of 100 patients with chronic CHF and 30 patients with non-CHF was measured by double antibody sandwich ELISA method. The correlation of APN,BNP levels and cardiac function were
analyzed. Results The plasma APN and BNP levels of patients with cardiac function II - IV significantly increased, compared with the control group and the difference was significant(P<0. 05); The plasma APN and BNP levels of CHF patients increased with the increasing of cardiac function classification, the differences were significant between different level of cardiac function(P<0. 05); After treatment, the plasma APN and BNP levels of patients with cardiac function II - IV were significantly decreased, compared with pre-treatment and the differences were significant P<0. 05). The plasma APN and BNP levels of patients with CHF showed a linear correlation with the cardiac function(r =0. 887 ,0. 984 ,P<0. 01). Conclusion The plasma APN and BNP levels of CHF patients are closely related to the severity of heart failure, the plasma APN and BNP levels can be used as clinical indicators for evaluating the therapeutic effect of patients with chronic CHF.
