Some members of staff have requested that quality checks should be carried out on work completed by the word processor operators. Please design a quality control form in 3 sections. The first section will be completed by the originator of the work, the second section by the operator who completes the work and the third section by the originator when he receives the completed work. The originator should then return the completed form to M Callender, Office Manager.

The form should include date submitted, name of originator, details of work to be done, any special instructions, date required, task completed by (add a note stating that this information should be in block capitals), was the work carried out satisfactorily, any comments the originator of the work wishes to add, space should also be left for signature and date. A suitable heading should be put on the form and an instruction to return the form.

Prepare the quality control form.
