This Design Contract ("Contract") is made effective as of [Date] by and between [Designer Name] ("Designer"), with an address at [Designer Address], and [Client Name] ("Client"), with an address at [Client Address].
1. Scope of Work
1.1 Designer agrees to provide the following design services to Client:
