溃疡性结肠炎患者结肠黏膜组织中LRG1、TGF-β和Smad2
表达水平及临床意义

【摘要】Objective To study the expression levels and clinical significances of LRG1,transforming growth factor β (TGF - β)and Smad2 in the colonic mucosa tissues of patients with ulcerative colitis (UC). Methods The most significant intestinal mucosa tissues of 85 pa-tients with UC in our hospital from March 2016 to November 2017 were selected as the study group,the normal intestinal mucosa of 52 reexamina-tion of normal healthy subjects after single polyp resection in the same period was used as the control group,the expression levels of LRG1,TGF- β and Smad2 in intestinal mucosa tissues was detected by immunohistochemical staining,the relationships between the expressions of LRG1, TGF - β and Smad2 in the mucous tissues of UC patients with the stage of the disease and the classification of endoscopy were analyzed,the ex-pression levels of
LRG1,TGF - β and Smad2 in the mucous tissues of different degree of disease during active stage,and the correlations between the expression of TGF - β and the expressions of LRG1 and Smad2 in the mucous tissues of the UC patients were analyzed. Results The positive expression rates of LRG1 and TGF - β in the intestinal mucosa tissues of the study group was
significantly higher than that of the control group (P < 0. 05). The positive expression rate of Smad2 was significantly lower than that of the control group (P < 0. 05). The LRG1 and TGF - βwere mainly expressed in the cytoplasm of normal mucosa tissues,and also expressed significantly in the cytoplasm and nucleus of the UC mucosa tissues,the Smad2 was mainly expressed in the cytoplasm and membrane of normal mucosa tissues and expressed in the cytoplasm of UC mucosa tissues;the positive expression rates of LRG1,TGF - beta in intestinal mucosa tissues of active UC patients were significantly higher than that of remission patients (P < 0. 05). The positive expression rate of Smad2 was significantly lower than that of remission patients (P < 0. 05). The positive expression rates of LRG1 and TGF - β in intestinal mucosa tissues showed the increasing trends with the increase of endoscopic grade of UC patients (P < 0. 05). The positive rate of Smad2 showed a decreasing trend with the increase of endoscopic grade of UC patients (P <0. 05). The positive expression rates of LRG1 and TGF - β in intestinal mucosa tissues of active UC patients showed the increasing trends with the aggrava-tion of the disease (P < 0. 05). The Smad2 positive expression rate showed a decreasing trend with the aggravation of the disease (P < 0. 05). The TGF - β expression in intestinal mucosa tissues of UC patients was positively correlated with LRG1 (r = 0. 632,P = 0. 000),and Smad2 was negatively correlated (r = - 0. 650,P = 0. 000). Conclusion The expressions of LRG1 and TGF - β in colonic mucosa tissue of UC patients were significantly increased,the expression of Smad2 was significantly decreased,which was related to the severity of the
disease,it may play an important role in the pathogenesis of UC and development of the disease.%目的研究溃疡性结肠炎(UC)患者结肠黏膜组织中富亮氨酸α2糖蛋白1(LRG1)、转化生长因子β(TGF-β)和Smad2表达水平及临
床意义.方法前瞻性采集2016年3月至2017年11月接受诊治的85例UC患者病变最显著的肠黏膜组织为研究组,同时期单发息肉切除后行结肠镜复查正常的52例健康者正常肠黏膜组织为对照组.采用免疫组织化学染色法分别检测肠黏膜组织
中LRG1、TGF-β和Smad2表达水平.分析UC患者黏膜组织中LRG1、TGF-β和Smad2表达与病情分期、内镜分级的关系.活动期不同病情程度黏膜组织中LRG1、TGF-β和Smad2表达水平.UC患者黏膜组织中TGF-β表达与LRG1、Smad2表
达的相关性.结果研究组肠黏膜组织中LRG1、TGF-β阳性表达率显著高于对照组(P<0.05),Smad2阳性表达率显著低于对照组(P<0.05);LRG1、TGF-β主要在正常黏膜组织细胞质中表达,在UC黏膜组织细胞质和细胞核中均有明显表达,Smad2主要在正常黏膜组织细胞质和细胞膜上表达,在UC黏膜组织细胞质中表达;活动期
UC患者肠黏膜中LRG1、TGF-β 阳性表达率显著高于缓解期患者
(P<0.05),Smad2阳性表达率显著低于缓解期患者(P<0.05);肠黏膜中LRG1、
TGF-β阳性表达率随UC患者内镜分级增高呈上升趋势(P<0.05),Smad2阳性表达率随UC患者内镜分级增高呈下降趋势(P<0.05);活动期UC患者肠黏膜LRG1、TGF-β阳性表达率随病情加重呈上升趋势(P<0.05),Smad2阳性表达率随病情加重呈下降趋势(P<0.05);UC患者肠黏膜组织中TGF-β表达与LRG1呈正相关关系
(r=0.632,P=0.000),与Smad2呈负相关关系(r=-0.650,P=0.000).结论 UC患者结肠黏膜组织中LRG1、TGF-β 表达量显著增加,Smad2表达量显著降低,与患者病情严重程度有关,可能在UC发病及病情发展过程中起重要作用.
