【摘要】Objective To investigate the changes of the serum T-lymphocyte subsets in peripheral blood and liver function in patients with chronic severe hepatitis B ( CSHB ) after treatment with entecavir .Methods Sixty patients with chronic severe hepatitis B were randomly divided into treatment group ( n =30) and control group ( n =30).The patients in both groups were treated by routine therapy ,however ,the patients in treatment group ,on the basis of control group ,were treated by entecavir for 24 weeks.The T lymphocyte subsets in peripheral blood and liver function were detected by flow cytometry before treatment and at 12th week and 24 week after treatment for both groups .Results After treatment the
CD4+subset and the ratio of CD4+/CD8+in both groups were increased in different degrees ,however ,CD8+subset was decreased ,as compared with that before treatment ( P <0.05),however,the changes in treatment group after treatment were more obvious than those in control group ( P
<0.05).The parameters about liver function in both groups were improved ,as compared with those before treatment,but the improvement degree in treatment group was more obvious than that in control group
( P <0.05 ). Conclusion Entecavir can obviously enhance cellular immune function and liver function of patients with chronic severe hepatitis B and can inhibit virus replication .%目的：观察恩替卡韦治疗慢性重型乙型肝炎患者外周血T细胞亚群及肝功能的变化。

方法60例慢性重型乙型肝炎患者随机分为治疗组和对照组，每组30例。

2组均接受综合治疗，治疗组在综合治疗基础上加用恩替卡韦抗病毒治疗，疗程为24周。

采用流式细胞仪检测2组治疗前及治疗第12周、24周外周血T细胞亚群，同时检测患者肝功能。

结果治疗后2组CD4＋亚群及CD4＋/CD8＋比值均有不同程度升高，CD8＋亚群降低，与治疗前比较差异有统计学意义（ P ＜0．05）；而治疗组治疗后各指标变化更为明显，与对照组比较差异有统计学意义（ P ＜0．05）。

治疗后2组肝功能各项指标均较治疗前有所改善，但治疗组改善程度优于对照组，差异有统计学意义（ P ＜0．05）。

结论恩替卡韦能够显著提高慢性重型乙型肝炎患者细胞免疫功能及肝功能，抑制病毒复制。
