单根弹性髓内钉治疗大龄儿童尺桡骨双骨折的临床研究

【摘要】Objective To investigate the clinical efficacy of single elastic stable intramedullary nailing in treat-ment of double fractures of radius and ulna in juvenile children. Methods A total of 25 juvenile children with double fractures of radius and ulna during January 2005 and June 2013 were recruited in this study, and Galeazzi, Monteggia, caput radii and distal metaphyseal fractures were obviated. The children were randomly divided into control group ( n=12), who were treated with double elastic stable intramedullary nails, and observation group (n=13), who were treated with a single elastic stable intramedullary nail, but 2 children in the observation group were excluded. The final clinical efficacy of the two groups was compared. Results The values of operation time, cost of hospitalization and incidence rate of complications in the control group were significantly higher than those in the observation group ( P>0. 05 ) , but there were no significantly differences in healing time of fracture and overall excellent rate of efficacy in the two groups (P>0. 05). Conclusion The rational use of single elastic stable intramedullary nail in treatment of older children with double fractures of radius has shorter operation time and takes cheaper hospitalization costs with similar efficacy compared with those by double
elastic intramedullary nails and lower incidence rate of complications.%目的探讨单根弹性髓内钉治疗大龄儿童尺桡骨双骨折的临床疗效. 方法选取本院2005年1月—2013年6月收治的25例尺桡骨双骨折大龄儿童,盖氏、孟氏、桡骨头和远端干骺端骨折被排除. 随机将患儿分为两组,12例患儿采用双根弹性髓内钉治疗作为对照组,13例患儿采用单根弹性髓内钉治疗作为观察组,观察组有2例患儿因故被排除,比较最终两组患儿的临床疗效. 结果对照组手术时间、住院费用、并发症发生率均高于观察组(P>0. 05),两组骨折愈合时间、术后疗效判定总优良率比较差异无统计学意义(P>0. 05). 结论合理使用单根弹性髓内钉治疗大龄儿童尺桡骨骨折的手术时间较短,住院费用较低,其治疗效果与使用双根弹性髓内钉治疗相似,且并发症的发生率可能更低.
