收养申请书

申请人：官珍福，男，44岁，住西洛乡洋海村雷家沟村民组。

于1998年1月结婚因女方生理有缺陷尚未生活，经夫妇双方多次协调同意收养子女一个，于xx年5月7日在平坝观音庙拾得女婴一个收养至今，现已上小学四年级。

现在我们夫妇双方都在酒厂上班每月收入3000余元左右，还有家庭付业在外，对其所收养的子女我们完全有能力把她抚养成人。
