输尿管镜在治疗妇科腹腔镜手术中输尿管损伤的卫生经济学研

【摘要】目的：探讨输尿管镜在治疗妇科腹腔镜手术中输尿管损伤的卫生经济学。

方法回顾性分析我市多家医疗机构1996年2月～2014年12月105例妇科腹腔
镜手术中输尿管损伤患者的病例资料，其中83例患者行经尿道输尿管镜双J管置入术（研究组），22例患者行开放性吻合手术（对照组），所有患者术后随访6
个月～1年，分析、总结并比较两组患者的疗效及卫生经济学。

结果研究组患者的住院费用低于对照组，两组间差异有统计学意义（t=7.82，P＜0.05），说明研究组患者的卫生经济学优于对照组。

两组患者术后均无并发症出现，研究组患者的手术时间和住院时间短于对照组，两组间差异有统计学意义（P＜0.05）。

结论经尿道输尿管镜下双J管置入术是处理妇科腹腔镜手术输尿管损伤的有效治疗方法，具有更经济、创伤小等优点。

%Objective To explore the health economics of ureteroscopy in the treatment of ureteral injury in gynecological laparoscopic surgery. Methods The case data of 105 patients with ureteral injury in gynecological laparoscopic surgery from February 1996 to December 2014 in our city much home medical institutions were retrospective analyzed, transurethral plasmakentic vaporization of ureteroscopy of 83 patients with double J tube surgery (research group),
22 patients line open anastomosis surgery (control group),all patients were
followed up for 6 months - 1 year postoperatively, the curative effect of two groups of patients and health economics were analyzed, summarized and compared.Results The hospitalization expenses of patients in research group were significantly lower than those of the control group, the differences between the two groups was statistically signiifcant
(t=7.82,P<0.05),The patient's health economics in the research group was superior to the control group. Two groups of patients had no postoperative complications, the team operation time and hospital stay in patients were signiifcantly shorter than those of the control group, the differences between the two groups was statistically signiifcant (P<0.05). ConclusionTransurethral ureteroscopy by the double J tube is the effective treatment of ureteral injury treatment of gynecologic laparoscopic surgery, more economic, affordable, and the advantages of small trauma.
