初步诊断: 蛛网膜下腔出血 Subarachnoid hemorrhage 三期高血压 Hypertension stage Ⅲ

鉴别: 无局灶症状的脑出血 各种脑膜炎 高血压脑病 精神症状突出者鉴别精神病
查体T37.9，P80/min, R18/min，BP150/85 营养中等，自主体位，一般状况可，心肺未见异 常。神经科查体：神清语利，颈抵抗（+），克 氏征（士），双瞳等大正圆，直径3mm，光反射 敏，口角右歪斜，伸舌居中，四肢肌力肌张力正 常，无共济失调，躯干四肢深浅感觉正常，生理 反射存在，病理反射未引出。
眼底发现玻璃体膜下出血,CSF均匀血性, 即可确诊

患者xxx,女,75岁,因“头痛，恶心，呕吐一天” 于2004.10.11入院。患者1天前无明显诱因出现 头痛，为额颞部剧烈疼痛，伴恶心呕吐，为少许 咖啡样液。当地测BP210/120，CT示“蛛网膜 下腔出血”，予“尼莫通，甘露醇，布瑞得”治 疗，用后生命征平稳，头痛稍缓解，转我科进一 步治疗。病程中饮食睡眠可，大小便正常。即往 高血压5年，8月前因脑出血在我院治疗好转后出 院。
病因:脑底部或脑表面的血管形 青年人 • 高血压、糖尿病引起的动脉瘤老年人 • 其它

体征: -脑膜刺激征, 绝大多数有出血 -运动感觉及颅神经异常出血部位(粗) -眼底改变: 淤血表现&积血特异

脑血管畸形:多为动静脉畸形,大脑前、中 动脉供血区的脑表面
