[摘要] 目的:探究彩色多普勒超声诊断下肢骨折后下肢深静脉血栓(DVT)的应用价值及其对肺动脉栓塞的预测价值。
方法:回顾分析84例下肢骨折后DVT患者超声诊断资料,所有患者先行X射线检查,再行彩色多普勒超声检查,分析比

[Abstract] Objective: To explore the application value of color Doppler ultrasound in diagnosing deep venous thrombosis (DVT) of lower limbs post fracture of lower limb and its predictive value for pulmonary artery embolism. Methods: The ultrasonic diagnosis data of 84 patients with DVT post fracture of lower limb were retrospectively analyzed. At the first step, all of patients underwent X-ray examination, and then they received the examination of color Doppler ultrasound. The examination results of two methods were analyzed and compared. And the statistic software SPSS21.0 was applied to analyze the predictive value of color Doppler ultrasound in diagnosing pulmonary artery embolism. Results: The total detectable rate of color Doppler ultrasound (80.95%) for DVT was significantly higher than that of X-ray (67.86%) (x2=4.489, P<0.05). And the sensitivity and specificity of color Doppler ultrasound (89.19% and 80.00%) were significantly higher than those of X-ray (68.92% and 60.00%) (x2=12.407, x2=9.523, P<0.05), respectively. As the results of color Doppler ultrasound examination, the vascular inner diameter of healthy limb was significantly larger than that of affected limb, and the blood flow velocity of healthy limb was obviously faster than that of affected limb. And the abnormality rate of vascular intima and dysfunction rate of valve of healthy limb (11.93% and 19.27%) were significantly lower than those of affected limb (62.39% and 57.80%) (x2=54.519, x2=31.339, P<0.05), respectively. 40 patients with arterial embolism were confirmed by venogram. And the detectable rate of pulmonary artery embolism by color Doppler ultrasound was 52.50%, which was significantly higher than that by X-ray (27.50%), (x2=13.020, P<0.05). Conclusion: Color Doppler ultrasonography has a higher application value in diagnosing DVT post fracture of lower limb, and it has a certain predictive value for pulmonary artery embolism, and it can provide reliable reference for clinical treatment.
