【摘要】目的将甘露醇与高渗盐水治疗急性脑出血患者对神经功能影响及疗效进行分析与探讨.方法选取自2013-01-2016-12间在广州医学院荔湾医院进行救治急性脑出血患者92例,并按照数字表达法,分为高渗盐水组与甘露醇组,每组均46例患者.高渗盐水组患者采用10%NaCl溶液治疗,甘露醇组采用20%的甘露醇进行治疗,观察两组患者神经功能缺损评分(NIHSS)、用药1h后颅内压(ICP)变化、药物效应持续时间以及不良反应(颅内压反跳,电解质紊乱)发生情况.结果高渗盐水组治疗后NIHSS评分,用药1h后ICP值、药物效应持续时间、颅内压反跳、电解质紊乱等明显优于甘露醇组,差异有统计学意义(P<0.05).结论急性脑出血患者应用高渗盐水治疗的临床效果明显,患者神经功能恢复好,降低颅内压的速度及效果均优于甘露醇,安全性较高.%Objective To investigate and analyze the effect of mannitol and hypertonic saline on nerve function of patients with acute cerebral hemorrhage.Methods 92 patients with acute cerebral hemorrhage who were treated in the hospital from January,2013 to December,2016 were divided into two groups according to digital number method,namely hypertonic saline group and mannitol group,with 92 cases in each group.Patients in hypertonic saline group were treated with 10% NaCl solution, and patients in mannitol group were given 20%
mannitol.Neurological deficit scores (NIHSS), intracranial pressure (ICP) at 1 hour after treatment, the maximum effect time and the incidence of adverse events (intracranial pressure rebound and electrolyte imbalance) were observed.Results NIHSS score, ICP value, maximum effect time, intracranial pressure rebound and electrolyte imbalance in hypertonic saline group after treatment were obviously higher than that in the mannitol group.The difference was statistically significant
(P<0.05).Conclusion Hypertonic saline is superior to mannitol in treatment of acute cerebral hemorrhage,promotes neurological function recovery,reduces intracranial pressure,and has a higher security.
