【摘要】Objective:To evaluate the clinical efficacy of lens capsules tension reducing capsulorhexis and conventional capsulorhexis in the white cataract phacoemulsification.Methods:Analyzed the clinical datas of 68 patients (76 eyes) with white cataract in our hospital from Jun.2011 to Jun.2016 retrospectively who were treated with cataract phacoemulsification combined with intraocular lens implantation,and all patients were divided into experiment group (n =34,36 eyes) and control group (n =34,40 eyes) according to the difference of intraoperative capsulorhexis.Experiment group treated with lens capsules tension reducing capsulorhexis during operation,while control group treated with conventional capsulorhexis during pared the integrity of the anterior capsule capsulorhexis,the degree of corneal endothelium edema,the intraocular lens deviation,uncorrected visual acuity and corneal endothelial cell count of the 2 groups.Results:A total of 34 eyes (94.44％) in the experiment group and 35 eyes (87.50％) in the control group successfully completed continuous annular capsulorhexis,,and there was no significant difference between the 2 groups (P＞0.05).The number of
patients Whose uncorrected visual acuity were greater than 0.5 increased 1 d～6 months postoperation,and which in the experiment group was significantly higher than that in the control group (P ＜0.05).Compared with the preoperative,1 d～ 6 months postoperation,the corneal endothelial cell counts in the 2 groups decreased significantly,but the difference between the 2 groups was not significant (P ＞ 0.05).The difference of intraocular lens deviation rates between the 2 groups at different times postoperation was not significant (P ＞ 0.05).At 1 day postoperation,the number of eyes without edema in the experiment group was more than that in the control group significantly,and the number of eyes with grade Ⅱ edema was less than that in the control gro up significantly (P＜0.05 or P＜0.01).Conclusion:The method of reducing capsulorhexis can make white cataract patients get better vision after surgery,reduce the degree of corneal edema,and have high clinical application value.%目的:探讨晶状体囊袋减低张力撕囊法及常规撕囊法在白色白内障超声乳化吸除术中的临床效果.方法:对来我院就诊的68例(76眼)白色白内障患者的临床资料进行回顾性分析,所有患者均采取白内障超声乳化术联合人工晶体植入手术进行治疗,根据术中撕囊方法的差异分为实验组(n=34,36眼)和对照组(n2 =34,40眼),实验组采用减低张力撕囊法,对照组采用常规撕囊法.比较两组患者术中晶状体前囊膜撕囊的完整性、术后角膜内皮水肿程度、人工晶状体偏移率、裸眼视力及角膜内皮细胞计数.结果:实验组中34眼(94.44％)顺利完成连续环形撕囊,对照组为35眼(87.50％),两组间差异无统计学意义(P＞0.05).从术后1 d～6个月两组裸眼视力大于0.5的患眼数量均呈升高趋势,且术后1个月及6个月实验组明显高于对照组(P＜0.05).术后不同时间两组人工晶状体偏移率差异无统计学意义(P＞
0.05).与术前比较,术后1 d～6个月两组角膜内皮细胞计数均明显降低,但两组间比较差异无统计学意义(P＞0.05).术后1d,实验组无水肿患眼数量明显高于对照组,Ⅱ级水肿患眼数量低于对照组(P＜0.05或P＜0.01).结论:减张撕囊法可使白色白内障患者获得更佳的术后视力,并降低角膜水肿程度,具有较高的临床应用价值.
