One day my dad just moved out and I had to go back and forth between my mom's house and his new apartment.I didn't understand why they couldn't all live together as a family anymore. My little brother Timmy was just a baby then so he didn't really know what was going on, but I felt like my world had been turned upside down.It was really hard splitting my time and belongings between two different homes. Sometimes I would forget my favorite stuffed animal or toy at one house and then feel really sad at the other house without it. I also didn't like going back and forth all the time - the driving made me carsick a lot. And sometimes my parents would argue when they had to see each other during the handoffs.I started having a lot of anxiety and trouble sleeping over at my dad's apartment because it didn't feel like home to me. I would cry a lot and say I didn't want to go there. This made me feel really guilty because I knew it hurt my dad's feelings. My mom had to take me to see a therapist to help me adjust to the new living situation.The therapist taught me some coping strategies like taking deep breaths when I felt upset. She also gave me a workbook where I could write down my feelings. Getting all my anxious thoughts out on paper instead of keeping them bottled up inside helped a lot. My parents also agreed to make the transitions more fun by letting me pick where we'd eat or letting me pick a gift from the dollar store.Over time, going between houses got easier as I got used to the routine. I made my dad's apartment feel more like home by keeping my favorite stuffed elephant and some toys there. My parents also eventually stopped fighting as much during the handoffs.While it was a really hard adjustment at first, I'm proud that I was able to get through that tough situation. I know it was really hard on my mom and dad too. But they made an effort to stay friends and co-parent us kids in a positive way. I've learned thateven when something scary or unwanted happens, you can get through it by talking about your feelings and taking it one day at a time.Another difficult thing I had to face was when I struggled to learn how to read in 1st and 2nd grade. Reading just didn't click for me like it did for some of the other kids. They seemed to pick it up so easily while I really had to work hard at sounding out words. I would get frustrated and embarrassed when kids would laugh at me for reading slowly or getting stuck.My teacher had to give me extra specialized instruction during free reading time while the rest of the class moved ahead.I worked with a tutor after school too which took up my playtime. My parents read me books over and over to help me practice. I felt like I was really behind everyone else.In the end though, all that extra work paid off. By the beginning of 3rd grade, I became a confident reader. Being able to read easily opened up a whole new world for me and is now one of my favorite things to do. I'm so glad my parents and teachers didn't give up on me when I struggled. Their patience and encouragement motivated me to not give up either.While reading was an academic challenge, I also dealt with a physical health challenge last year. When I was 8 years old, Ibroke my arm after falling off the monkey bars at the playground. Having to wear a cast for 6 weeks and not being able to use my dominant arm was so hard! I couldn't dress myself, brush my teeth, or even eat certain foods. My mom had to help me do everything.Writing was the most difficult since I'm right-handed and couldn't really move that arm. Taking notes in class was slow and messy. Thankfully, my teacher let me type assignments on the classroom computer. But I still felt like I was behind and working twice as hard as everyone else. Things like tying my shoes or zipping up my jacket were impossible. I felt kind of useless!My cast also meant I couldn't go swimming or play sports that summer which I was really bummed about. Instead of my usual active outdoor kid activities, I had to find other quieter things to do for fun like reading, playing video games, or watching movies. It was boring at times but I knew it was temporary until my arm healed.When I finally got the cast off after 6 long weeks, it was like being free again! My arm was skinny and weak at first from not using it, but it felt so good to have full mobility again. I didn't take little things for granted anymore like being able to brush myown hair. Overcoming that challenge also taught me how to be more patient and adapt to limitations.So those are three pretty big things I've struggled with in my childhood so far - my parents' divorce, learning difficulties with reading, and breaking my arm. But with help from my family, teachers, doctors and just general perseverance, I was able to pull through. 甲泼尼龙(methylprednisolone,MP)在最新脊髓损伤指南中的变化由美国神经外科医师大会(Congress of Neurological Surgeons,CNS)和美国神经外科医师协会(American Association of Neurological Surgeons,AANS)共同制定的最新Guidelines for the Management of Acute Cervical Spine and Spinal Cord Injuries于2013年3月发表于Neurosurgery杂志。

旧版指南[1](2002年)中指出:推荐急性脊髓损伤(Spinal Cord Injuries,SCI)在24或48小时内使用甲泼尼龙(methylprednisolone,MP),只要医生认识到使用这些药物导致损害的几率高于其使用获益(附录1)。

指南对该说法的Recommendation等级为“Opinion”(附录2),表示为“Reflection of unclear clinical certainty”。

新版指南中[2],对急性SCI治疗的24或48小时内不推荐使用MP,并提醒临床医生MP的该用法并没有得到FDA的批准。

指南对该说法的Recommendation指数为“Level I”(附录2),表示“Reflection of a high degree of clinical certainty”。

新版指南指出,目前没有一类或者二类的临床研究证据(Class I、Class II、Class III,共三类,Class I确定性最高,依次递减,详见附录2)支持MP对急性SCI治疗有益,只有部分三类证据报道了MP对SCI的神经保护作用。

但此类研究都有局限性:样本数量有限或者报道的数据不完整。

并且病人出现运动功能恢复而感觉功能未恢复或者相反的情形,提示病人的恢复可能只是随机事件。
