UNIT6What does it feel like to help dying patients through their final days? Experience it through the eyes of hospice nurse Jill Campbell, who does her job with grace, compassion, and gratitude.1.Outside, it's noisy on this busy block of row houses in Baltimore. But inside one tidy living room, all is quiet except for the sound of a woman's raspy breathing. The patient is huddled in an easy chair under a handmade pink-and-blue afghan, a knit cap on her head and booties on her feet. She has trouble staying warm these days. Her cancer has returned with a vengeance and she has only a few weeks to life. Hospice nurse Jill Campbell kneels down beside her patient, listens to her breathing, and then checks her blood pressure. Campbell has already hauled in oxygen tanks, showed family members how to work them, organized the medicine, and assessed how her patient has been eating and sleeping.2.But now is a moment to connect one-on-one. Campbell wraps her hands aro und the woman’s hands and rubs them together to warm them. She looks into her face. “are you feeling a little better?” she asks softly.3.Getting to know her patients and helping them through the toughest time of their lives is what Campbell, 43, appreciates most about being a hospice nurse. “I don’t know of another position where you can do more for people,” she says.4.Her patients have all been told that they have six months or less to live. Rather than continue with often-difficult or painful treatments that probably won’t extend their lives, they have decided to stop trying for a cure. Instead, with the help of hospice care, they’ll focus on comfort and on living whatever they have left of their lives to the fullest ---usually in their own home.5.Being able to die at home is a major part of the appeal of hospice, but patients and family members may not see it that way at first. “A lot of people still view hospice as giving up and letting the disease in,” says Campbell. That’s why the decision to c all in hospice care can be an incredibly difficult one for a family to make. Once they do, though, most patients and their families soon understand the value of having a team of dedicated professionals---including social workers, health aides, chaplains, and nurses---work together to provide not only physical but also emotional and spiritual support.
PART 1Good morning everyone, my name is CWP and I’m from Class One. Today I’m going to talk about hospice care with you.As we all known, everyone is bound to be going old, which like leaves fell down from the tree in Autumn. You will feel painful in the end of life, like losing the ability to eat your own food, breathless, and dying. Unfortunately, may be you will suffer from cancer or other incurable diseases in the last stage of life. There is no doubt that you have two choices to face the unavoidable trouble, one is trying to prolong a painful existence , the other one is choosing a decent quality of life in the last phases of an incurable disease, which is also called hospice care.(In the traditional idea,) Some relatives of the patient prefer to the first choice because they want to express their love to him or her by begging doctors to try their best to prolong the patient's life, even the chemotherapy(化疗[ˌki:moʊˈθerəpi]) will bring some physical, mental, emotional and spiritual pain to the patient, they think it is the best way to help the sick, but in fact, what they want to do is just comfort themselves totally. In some way, it’s not only make the patient even more painful, but also waste the medical resources. In fact, most patients actually don’t need the rescuing doctors and nurses, the cold medical apparatus and instruments. What they really want is to meet with their relatives. After the confirming of the disease for which no cure has been found, perfect life nursing, psychological counseling and pain nursing is better for the patients. So,The objective of hospice care is to help the incurable patients spend the last life with no regret ,no pain, no fear.In our opinion, we agree with the second choice -- hospice care.Do you know what is hospice care?OK, let me tell you!Hospice care is a type of care and philosophy of care that focuses on the palliation of a chronically ill, terminally ill or seriously ill patient’s pain and symptoms, and spiritual needs. Why we prefer the hospice care? There are three reasons.Firstly, it can help patients overcome psychological problems and thespirit of fear, making patient calm to face death.Secondly, it can control pain and symptom. The objective of pain and symptom control is to help the patients to achieve comfort while allowing them to remain in control of their life. This means that side effects are managed to ensure that they are as free of pain and symptoms as possible, yet still alert enough to make important decisions. Thirdly, it can help patients’ family take some fatigue and stress away. In other word, it can give family of spiritual support and encouragement, and help them get through this moment of separation. It’s a good way to remove ineffective treatment which brings huge cost for family and pain of body and mind for patients. How to provide the hospice care? Now I am fortunately to share some ways to you.1.Reduce the patient’s physical and mental symptoms.2.Take treatment following patient’s desire to maintain his dignity.3. Avoid undue trauma treatment.4.Provide the patient and his family plenty of time to get along witheach other while the patient can communicate with people.5.Try to give patient the best quality of life.6.Reduce the burden of the family’s medical expenses and tell patient how much the professional nursing will cost.7.Provide the family for the deceased help for the funeral service.OK, after listening what I have introduced to you, have you learned some further detail about the hospice care? I think your answer is “yes” .PART 2There is no doubt that there are some hospices all over the world, and it’s no exception for Guangzhou. Before I share some information with you, let’s watch a video first.VIDEO’S TIMEAfter watching this video, do you have some understanding about China? And what have you got? Should I summery this video for you in Chinese?I think the answer is yes.\the classmate will answer you “yes”.

We may come to realize that the most important things in life are not material possessions or achievements, but rather the relationships we have with others and the impact we have made on the world. In our final moments, we may find ourselves thinking about the people we love and the memories we have shared with them. We may come to understand that love is the most powerful force in the universe, and that it is what gives our lives true meaning and purpose. As we face our mortality, we may also come to appreciate the beauty of the world around us. We may find solace in the simple pleasures of life, such as the warmth of the sun on our skin or the sound of the ocean waves. We may realize that the world is a wondrous place, and that we are lucky to have experienced it.In our last days, we may also find ourselves thinking about the legacy we will leave behind. We may come to understand that the impact we have made on the world will live on long after we are gone, and that it is important to make the most of the time we have left.As we approach the end of our lives, we may feel a sense of peace and acceptance. We may come to understand that death is a natural part of life, and that it is nothing to be feared. We may find comfort in the knowledge that we have lived a full and meaningful life, and that we are ready to move on to whatever comes next.In the end, the most important thing we can take away from our final days is a sense of gratitude. We may come to realize that we are grateful for the experiences we have had, the people we have loved, and the life we have lived. And as we take our last breaths, we can find peace in knowing that we have truly lived.。
As a healthcare professional, I believe that end-of-life care is an essential part of our duty to our patients. Providing comfort and support to patients and theirfamilies during this difficult time is crucial. It is a time when patients need not only medical care, but also emotional and spiritual support.I remember a patient I cared for who was terminally ill with cancer. She was in a lot of pain and was struggling to come to terms with her diagnosis. I made sure to spend time with her, listening to her fears and concerns, and providing reassurance. I also involved the palliative care team to help manage her pain and symptoms effectively. It was important for me to ensure that she was comfortable and that her family felt supported as well.In addition to medical care, it is important to addressthe emotional and spiritual needs of the patient. I remember another patient who found comfort in reading religious texts and having discussions about faith. I made sure to provide her with the resources she needed to feel spiritually fulfilled during her final days.I believe that end-of-life care is about treating the whole person, not just their physical symptoms. It is about providing comfort, dignity, and respect to the patient and their loved ones. It is a time when communication and empathy play a crucial role in providing the best care possible.

Asked where they would like to spend their last days, Americans almost always say at home, surrounded by people they love. In real life, though, only one in five achieves that. More than30% die in a nursing home, where almost no one wants to be, and over half end up in ahospital, often in an intensive-care unit, heavily sedated and attached to life-savingequipment until their doctors give up the battle.
Death is a difficult subject for anyone, but Americans want to talk about it less than most. They have a cultural expectation that whatever may be wrong with them, it can be fixedwith the right treatment, and if the first doctor does not offer it they may seek a second, thirdor fourth opinion. Litigation is a constant threat, so even if a patient is very ill and likely todie, doctors and hospitals will still persist with aggressive treatment, paid for by the insureror, for the elderly, by Medicare. That is one reason why America spends 18% of its GDP onhealth care, the highest proportion in the world.

<Finding Care at the End of Life>文章大概1-2段:General introduction of cares at the End of Life3-5段:Differences between terminal cares provided at hospitals and nursing homes6-8段:Terminal cares, provided at home, shall have some other helps too.9-14段:Introduction of palliative cares ( meanings, applicable conditions, provider, sponsor )15-19段:Characters of hospice cares ( no cutative treatment, not permanent ). Decades ago, most people died at home, but as medication advances, more and more people choose hospice care at the end of life.If given chances, each person and his/her family should consider which type of care is the most suitable. And how many types of care do we have?We have cares at hospitals, nursing homes, homes and can get palliativecares, which is similar to hospice cares. The advantages of choosing cares at hospitals are that there is always medical staff available who know what needs to be done for someone who is dying. But this may have less personal cares and privacies for patients,so the nursing homes services are acceptable because they can provide cares more privately.Choosing cares at homes are also good options.Because this can let patients feel warmer. In order to relieve the burden of patients’ family, some other supports are necessary, such as medical conselor, visiting nurses, special equipments(commodes) and so on.In addition to this, palliative cares are meaningful sometimes.Palliative cares now often refer to a comprehensive approach to improving patients’ quality of lives who are living with potentially fatal diseases, through using both curative andalleviating treatments meanwhile if it is useful. At some point, however, curative treatment may no longer make sense, even make the patients more uncomfortable.Hospice is designed for this situation.Different from palliative cares, attempts to cure illness are stopped. But it doesn’t mean discontinuing all treatments. On the base of staff participating in palliative care teams, spiritual advisors and trained volunteers are available too.Choosing hospice doesn’t have to be a permanent decision. You can change your mind at any moment.。
