Conclusion:
(1) The ratio of heterotypic lymphocytes in children infected with infectious mononucleosis was significantly higher than that in normal children.
儿童传染性单核细胞增多症异型淋巴细胞及淋巴细胞亚群的研究
摘 要
目的:
探讨儿童传染性单核细胞增多症异型淋巴细胞及淋巴细胞亚群的变化及其意义。
方法:
选取2016年12月-2017年12月青岛大学附属医院儿科治疗的41例传染性单核细胞增多症作为实验组,同期选取60例健康儿童作为对照组。所有受试者取空腹静脉血,提取 DNA后采用荧光定量 PCR测定全血EBV的拷贝数,并进行异型淋巴细胞的比值的测定及血细胞计数分析,采用流式细胞术测定淋巴细胞亚群(CD3+T淋巴细胞、 CD4+T淋巴细胞、CD8+T淋巴细胞、 NK细胞、B细胞等),比较两组的差异,进行异型淋巴细胞与淋巴细胞亚群之间的相关性分析。采用SPSS19.0统计软件包进行数据处理。计量资料采用均数±标准差(s)表示,符合正态分布及方差齐性的组间计量资料比较采用t检验,不符合者采用非参数检验。计数资料采用百分数(%)表示;组间计数资料采用 x2 检验;相关性分析采用Spearman相关性检验。P≤0.05差异有统计学意义。
Objective:
To explore the change of heterosexual lymphocytes andlymphocyte subsets and their clinical value in infectious mononucieosis children.
Methods:
41 cases of infectious mononucleosis in the Affiliated Hospital of Qiingdao University, December 2016 -2017 and December were selected as the experimental group, and 60 healthy children were selected as the control group at the same time. All the subjects were taken fasting venous blood. After extracting DNA, the number of EBV in whole blood was measured by fluorescence quantitative PCR, and the ratio of the hetero lymphocyte and the blood cell count were measured. The lymphocyte subsets (the proportion of CD3+T lymphocyte, the proportion of CD8+T lymphocyte in CD4+T lymphocyte proportion, the proportion of CD8+T lymphocyte in CD4+T) were measured by flow cytometry. CD56+ CD16+ ratio, etc., to compare the differences between the two groups, and to analyze the correlation between atypical lymphocytes and lymphocyte subsets. SPSS19.0 statistics software package is used for data processing. The measurement data were expressed by mean number of standard deviation (s). T test was used to compare the data between normal distribution and homogeneity of variance, and nonparameter test was used for the non conforming. Counting data were expressed by percentage (%); the count data between groups were tested by x2 test; correlation analysis was conducted by Spearman correlation test. The difference between P ≤ 0.05 was statistically significant.
(4)传染性单核细胞增多症儿童外周血异型淋巴细胞数量可以提示机体免疫活化的程度。

(1)两组儿童在性别、年龄、身高和体重上无明显统计学差异( P﹥0.05),但是实验组儿童的体温较对照组儿童显著升高( P﹤0.05)。(2)实验组儿童的异型淋巴细胞比值为(8.25±2.35)%,而对照组儿童的异型淋巴细胞比值为(0.82±0.25)%, 差异具有统计学意义( P﹤0.05)。(3)实验组儿童CD3+T淋巴细胞比例和 CD8+T淋巴细胞比例较对照组儿童显著升高( P<0.05), 而 CD4+T淋巴细胞比例、 CD4+/ CD8+、 CD56+ CD16+比例较对照组儿童显著降低( P<0.05)。(4)传染性单核细胞增多症异型淋巴细胞比例与白细胞总数(r=0.524,P=0.015)、淋巴细胞计数(r=0.661,P=0.01)呈正相关。(5)异型淋巴细胞与CD3+T淋巴细胞计数(r=0.497,P=0.036)、CD8+T淋巴细胞计数(r=0.517,P=0.028)及CD56+CD16+计数(r=0.511,P=0.030)呈正相关,与CD4+/CD8+(r=-0.527,P=0.025)呈负相关;与CD4+T淋巴细胞计数(r=-0.449,P=0.061)无相关;异型淋巴细胞与T淋巴细胞总数(r=0.627,P=0.009)、活化T细胞(r=0.612,P=0.007)呈正相关,与调节T细胞(r=-0.578,P=0.015)呈负相关,与活化B细胞(r=-0.249,P=0.319)无相关。(6) 实验组患儿EBV-DNA滴度与异型淋巴细胞(R=0.190,P=0.239)无相关。

(5) heteromorphic lymphocytes were positively correlated with CD3+T lymphocyte count (r=0.497, P=0.036), CD8+T lymphocyte count (r=0.517, P=0.028) and CD56+ CD16+ count (r=0.511,P=0.030), and negatively correlated with CD4+/CD8+ (r=-0.527,); heteromorphic lymphatic finer The cells were positively correlated with the total number of T lymphocytes (r=0.627, P=0.009) and activated T cells (r=0.612, P=0.007), and negatively correlated with the regulation of T cells (r=-0.578, P=0.015), and was not related to the activation of B cells (r=-0.249, P=0.319). (2) There was obvious T lymphocyte immune disorder in EBV infected children with infectious mononucleosis, the increase of CD8+T lymphocyte, the decrease of CD4+T lymphocyte proportion and the increase of CD3+T lymphocyte proportion.
结论:
(1)传染性单核细胞增多症儿童的异型淋巴细胞比值较正常儿童显著增加。
(2)传染性单核细胞增多症儿童存在明显T淋巴细胞免疫紊乱,CD8+T淋巴细胞明显增多,CD4+T淋巴细胞比例明显下降,CD3+T淋巴细胞比例上升。
(3)传染性单核细胞增多症异型淋巴细胞的比例与外周血白细胞总数、淋巴细胞总数、T淋巴细胞计数、活化T细胞呈正相关,与CD3+T淋巴细胞计数、CD8+T淋巴细胞计数以及CD4+ /CD8+可能存在相关。
