【摘要】目的:探讨NGAL、E-cadherin和MMP-9在结肠癌复发与转移中的作用及与预后的相关性.方法:回顾性分析天津医科大学附属肿瘤医院2000年1月～2004年12月227例结肠癌患者的临床资料,总结其病理特征及随访结果,并将227例病例对应的石蜡标本制作成组织芯片,其中复发与转移组83例,无复发转移组144例,用免疫组织化学法检测结肠癌组织中NGAL、E-cadherin和MMP-9的表达,分析其与复发转移及预后的关系.结果:复发转移组NGAL、MMP-9表达高于无复发转移组(P=0.032;P=0.000),复发与转移组E-cadherin 表达低于无复发转移组(P=0.030).NGAL、MMP-9 阳性组平均生存时间分别为(47.427±2.505)个月、(48.081±2.672)个月,中位生存时间分别为37、35个月,阴性组平均生存时间分别为(65.714±3.616)个月、(69.598±2.073)个月,中位生存时间分别为77、75个月,NGAL、MMP-9阳性组生存期较阴性组缩短,差别具有统计学意义
(P=0.000,P=0.010).E-cadherin阳性组平均生存时间为(61.587±2.805)个月,中位生存时间为61个月,E-cadherin阴性组为(39.535±2.946)个月,中位生存时间为23个月,E-cadherin阳性组生存时间较阴性组延长,有统计学意义(P=0.000).结论:NGAL、MMP-9过度表达,E-cadherin表达缺失可能与结肠癌复发、转移,不良生存预后存在相关性.%Objective: To investigate the effects of NGAL, MMP-9 and E-cadherin on the recurrence and metastasis of colon carcinoma and
its correlation with clinical prognosis.Methods: Clinical characteristics and prognosis of 227 colon carcinoma patients diagnosed between 2000 and 2004 were reviewed.The paraffin embedded tissue samples were selected and tissue microarray blocks were made ( 83 with recurrence and metastasis and 144 without recurrence and
metastasis ).Immunohistochemical staining was used to evaluate the expression of NGAL, MMP-9 and E-cadherin in colon carcinoma and its correlation with recurrence and metastasis in colon carcinoma was analyzed.Results: The abnormal expression of NGAL and MMP-9 in the recurrence and metastasis group was higher than that in the group without recurrence or metastasis ( P = 0.032, P = 0.000 ).The positive ratio of E-cadherin in the recurrence and metastasis group was lower than that in the group without recurrence or metastasis ( P = 0.030 ).The mean survival was 47.427 ± 2.505 months in patients with positive expression of NGAL and 48.081 ± 2.672 months in patients w ith positive expression of MMP-9.The median survival time was 37 and 35 months, respectively.The mean survival time was 65.714 ± 3.616 months in patients without NGAL expression and 69.598 ± 2.073 months in patients without MMP-9 expression.The median survival time was 77 and 75 months, respectively.The survival of patients with positive expression of NGAL and MMP-9 was shorter than that of patients with negative expression of NGAL and MMP-9 ( P = 0.000, P = 0.010 ).The mean survival time and median survival time of patients with E-cadherin expression were 61.587 ± 2.805 months and 61 months, significantly higher than those in patients without
E-cadherin expression ( P = 0.000 ), which were 39.535 ± 2.946 months and 23 months, respectively.Conclusion: The overexpression of NGAL and MMP-9 and decreased expression of E-cadherin may play critical roles in the recurrence and metastasis of colon carcinoma and indicate a poorer prognosis.
