多层螺旋CT血管造影对下肢动脉硬化闭塞症的诊断价值

【摘要】目的：探讨多层螺旋CT血管造影对下肢动脉硬化闭塞症的诊断价值。

方法选取2012年10月至2015年7月滁州市第一人民医院血管外科诊治的下肢
动脉硬化闭塞症患者55例，均接受下肢动脉DSA及多层螺旋CT血管造影。

观察多层螺旋CT血管造影对下肢动脉病变的显示情况。

以数字减影血管造影（DSA）为金标准，计算多层螺旋CT血管造影对下肢动脉闭塞的诊断效能。

结果多层螺旋CT血管成像共显示病变血管529段，其中轻度狭窄379段、中度狭窄100段，
重度狭窄32段，闭塞18段。

以DSA为标准，多层螺旋CT血管造影与之诊断符合519段。

其中对血管狭窄高估4段，对血管狭窄低估6段。

一致性检验显示多
层螺旋CT血管造影与 DSA对下肢动脉闭塞症诊断情况Kappa值＝0.984。

多层
螺旋CT血管造影对下肢动脉闭塞的诊断灵敏度为94.1％、特异度为99.8％、阳
性预测值为88.9％、阴性预测值为99.9％、准确度为99.7％。

多层螺旋CT血管
造影对下肢动脉中度以上狭窄的诊断灵敏度为96.6％、特异度为99.2％、阳性预
测值为95.3％、阴性预测值为99.4％、准确度为98.8％。

结论多层螺旋CT血管
造影在下肢动脉硬化闭塞症诊断中具有较高的诊断价值。

%Objective To investigate the value of multi-slice spiral CT angiography in the diagnosis
of lower extremity arterial occlusive disease.Methods Total of 55 cases of lower extremity arterial occlusive disease in Chuzhou First People′s Hospital from Oct.2012 to Jul.2015 were selected as the research object,
which all received DSA and multi-slice spiral CT angiography in lower extremity arteries.The lower extremity arterial disease condition was observed by multi-slice spiral CT angiography.With digital subtraction angiog-raphy(DSA) as the gold standard,the effect of multi-slice spiral CT angiography in the diagnosis of lower extremity arterial occlusion was calculated.Results Multi slice spiral CT angiography detected 529 segments of lesions,including 379 segments of mild stenosis,100 segments of moderate stenosis,32 cases of severe ste-nosis, 18 blocks.With DSA as the standard,the multi-slice spiral CT angiography identified 519 segments, including 4 cases of overrating of the stenosis degree,and 6 cases of underestimating the stenosis.Consistency test showed that the DSA value of the diagnosis of lower extremity arterial occlusive disease with multi-slice spiral CT angiography Kappa was 0.984.The diagnostic sensitivity was 94.1%,specificity was 99.8%,posi-tive predictive value was 88.9%, negative predictive value was 99.9%, diagnostic accuracy rate was 99.7%.For medium lower extremity arterial occlusive disease,the diagnostic sensitivity was 96.6%,speci-ficity was 99.2%, positive predictive value was 95.3%, negative predictive value was 99.4%, diagnostic accuracy rate was
98.8%.Conclusion Multi-slice spiral CT angiography has high diagnostic value in the diagnosis of lower extremity arteriosclerosis occlusive disease .【总页数】3页(P4155-4157)
