材料试验报告Covering Letter of Material Testing Reports

Objective：
To provide a simple approach for suppliers to combine/transfer test reports to Quanta standard report format. It also acts as an alternative solution for suppliers to save the cost for requesting testing labs to combine/re-issue reports to Quanta format.
Scope：
1. In your original report, there is no manufacturer part number listed.
2. When new part number occurs, the renewal of applicable part numbers on your report.
3. The need to combine several minor test reports into a complete test report.
4. To check the RoHS compliancy of materials used in manufacturing process.
Instructions：
1. Supplier Company Name -
> Fill in the full company name and date
2. Supplier Company Address -
> Fill in the company address
3. Supplier Contact -
> Fill in the contact person, name, title, phone#, email and signature
4. Product Description -
> Give a brief description for the product name and function
5. Quanta Part Number -
> List the corresponding manufacturer P/N to the report
6. Material used in manufacturing process –
> Fill in manufacturing subsidiary (for example: solvent, cleaning agent, electroplate solution, etc.)
and declare its RoHS compliancy
7. Test Reports -
> To combine several reports into one final report for Quanta, please list all test positions, test labs, report numbers, and report dates
8. Description for Failed Parts -
> Explain the reason of failed parts/failed materials and provide appropriate corrective action plan
File combination：
Quanta’s environmental website allows users to upload only one test port for each part number, thus file combination is necessary when you have several relevant testing reports corresponding to a part number. Please insert this covering letter and all applicable into a PDF file before upload it to the website.
