1.《博士后申请表》（系统在线填写，电子版+纸版一份）

2.《博士后进站审核表》（纸版一份）
Qualification Form of Post-doctoral Fellowship Program (printed version is required) 3. 《博士后科研流动站设站单位学术部门考核意见表》（电子版）
The Suggestion Form of Academic Department of Post-doctoral Fellowship Institution (E-version of the form is required)
4.《首都医科大学博士后人员进站考核表》（电子版）
Evaluation Form of Post-doctoral Fellowship Program of Capital Medical University (E-version of form is required)
5. 博士学历及学位证书或博士论文答辩决议
The Ph.D Degree Certificate or the Final Decision of Competitive Examination for the Equivalent Degree
6. 博士后工作协议书
Work Agreement of Post-doctoral Fellowship Program
7. 身份证原件及复印件
An Original and A Copy of Permanent Residence ID
8. 体检表(指定：朝阳医院西院<北京石景山区京原路5号>)
Physical Examination Certificate (Authorized Institution: Beijing Chaoyang Hospital Jingxi Hospital Area. Add: No.5, Jingyuan Road, Shijingshan District, Beijing)
9. 结婚证原件及复印件
An Original and A Copy of Marriage Certificate
10. 申请人子女的独生子女证（或出生医学证明）原件及复印件
