没有稳定输送的新鲜血液,没有血液携带的氧气,人类大脑很快就会受损。

4分钟内,脑细胞就会因缺氧开始死亡,从而导致严重的脑部损伤。

再过几分钟,大脑将遭受毁灭性损坏。

这就是这难题的症结所在。

心脏停止工作不能超过4分钟——用来修复心脏问题的时间非常之少。

除非找到一种解决方案,否则打开胸腔进行手术是不可能的。

后来两项先进技术给心内直视手术指明了道路。

一项技术是降低体温法,人工实现体温降低。

人体组织在寒冷的时候需要很少的氧气。

例如,一个“冷冻”的大脑与一个体温华氏98.6度的正常大脑相比,仅需要其一半的氧气量。

也就是说,大脑在缺少新鲜血液供应的情况下,可以维持超过4分钟的手术时间,而不会受损。

1950年,加拿大多伦多大学的威尔弗雷德·G·比奇洛博士有了这一重大发现,他立刻预见到降低体温可能让心脏手术有机会在“缺血无血的心脏上实行。

”通过降低体温法,外科医生可以将流向心脏的动脉夹紧,阻断血液流进心脏。

他可以打开心脏,将其中的血液排尽,在一个“干燥的场所”实施超过5分钟的手术。

两年后,明尼苏达州立大学的弗洛伊德·约翰·刘易斯博士利用降低体温法尝试了心内直视手术。

手术后的第二天,五岁的患者就能够走路了,十天以后她就允许出院了。

心脏手术的未来看上去前景光明。

但在长时间和复杂的心脏手术中使用降低体温法就不那么安全了。

体温降低的情况下,心脏有时会失去常规的跳动。

心脏会无规律跳动,间隙性跳动或者无用性颤动。

如果心跳不能复原心脏不能恢复正常跳动,这种无规律跳动可能导致死亡。

接着,心内直视手术的第二项主要前沿技术:一台可以代替心和肺工作的机器。

心肺仪像心脏一样输送血液,像肺一样去除血液中的二氧化碳,补充血液中的氧气。

事实上在手术中是它在为患者呼吸以及帮助循环血液。

这台机器给心脏手术带来了彻底的革命。

很多医生的研究对这一复杂装置的研发做出了贡献,但是其中一位功不可没。

他就是费城杰斐逊医学院的约翰·H·吉本博士。

作为心肺仪之父,他对其中难题的研究和探索接近20年之久。

1953年5月,吉本博士相信他的心肺仪可以投入使用了。

他的第一个患者是一位患有严重心脏疾病的18女孩。

在她的右心房和左心房之间有个半个硬币五角硬币大小的洞。

仪器“连在”女孩身上接近半个小时,吉本博士和他的团队在此期间缝合了这个洞。

这是人类历史上心肺在外科手术过程中第一次被人工装置所取代。
