患者病情评估制度及操作规范为了保证医疗质量,保障患者生命安全,使患者从进院开始就能够得到客观科学的评估,医生能够做出详细科学的治疗计划,当病情变化的时候能够及时调整修改治疗方法,使患者得到科学有效的治疗,特制定我院患者病情评估制度。

1.医师对接诊的每位患者都应进行病情评估。

2.医师对门诊病人进行评估时要严格掌握住院标准,严格按照患者的病情作为制定下一步治疗的依据,严禁将需住院治疗的病人进行门诊观察治疗。

假如门诊医生决定需要住院的患者拒绝入院治疗,医生必须做好必要的知情告知,详细告知患者可能面临的风险,并签署患者的名字。

3.病人入院后,主管医师应对病人全面情况进行评估,包括病情轻重、急缓、营养状况等做出正确的评估,做出正确的诊断,参照疾病诊治标准,制定出经济、合理、有效的治疗方案并告知患者或者其委托人。

4.临床医生除了对患者的病情进行正确科学的评估,还应该对患者的心理状况作出正确客观的评估,全面衡量患者的心理状况,对有可能需要作心理辅导的患者进行必要的登记并作记录,随时请心理卫生科医生给予必要的心理支援。

5.对病人在入院后发生的特殊情况的,应及时向上级医生请示,再请科主任共同再次评估。

必要时可申请会诊,再集体评估。

6.病人在入院经评估后,本院不能治疗或治疗效果不能肯定的,应及时与家属沟通,协商在本院或者转院治疗,并做好必要的知情告知。

7.所有的评估结果应告知患者或其病情委托人,病人不能知晓或无法知晓的,必须告知病人委托的家属或其直系亲属。

一、临床路径定义与内容临床路径是医生、护士和其他人员共同制定的针对某种诊断或手术所做的最适当的有顺序性和时间性的整体服务计划,是标准化诊疗护理流程,是医院实施实时质量管理的最简单易行的方式。

目的是使患者获得最佳的服务、减少康复的延迟和资源的浪费。

它主要是针对ICD码对应的病种或某种手术使用标准化的、综合多学科的过程来调整医疗行为,对病人的诊断,包括多种检查、治疗及护理要依据预先指定的基于时间或治疗结果的流程表顺序进行,在规定的时间、预算的费用内达到预定的治疗结果。

临床路径的内容包括预期结果、评估、多学科的服务措施、病人与其家人的相关教育、会诊、营养、用药、活动、检验与检查、治疗和出院计划以及变异的记录等内容。

二、总体目标通过临床路径管理实现医疗服务诊疗护理常规的标准化,提高工作效率和内涵质量。

通过明确病种的诊疗护理操作规程,使医护人员行为规范化、标准化,有效避免乱开药、滥检查等过度治疗现象,同时增进医患沟通,建立和谐医患关系,合理使用医疗资源,控制非必要医疗支出。

三、指导思想深入贯彻落实科学发展观,坚持“以病人为中心”,贯彻国家基本药物制度,进一步规范临床诊疗行为,不断提高医疗质量和效率,保障医疗安全,为人民群众提供安全、有效、方便、价廉的医疗服务。

四、临床路径实施(一)成立组织,明确职责1.成立临床路径管理领导小组组长:李维义副组长:李普成员:陶生学梁彬强生毅黄庆英张凤泽宋占杰韦耀刚葛桂芝马建伟张明兰王波陶育香张芙莉周钢胡训张建刚徐志明刘积学马长寅医院成立临床路径管理办公室,设在医务科。

临床路径管理办公室主任:陶生学(兼)。

2.成立临床路径管理评价专家小组组长:李普副组长:陶生学梁彬成员:马建伟韦耀刚宋占杰葛桂芝张芙莉王波张明兰陶育香医务科负责组织临床科室科主任修订临床路径,护理部负责组织护士长修订护理路径。

医务科、护理部负责临床路径管理的综合考评和相关统计工作,病案室负责相关病案信息收集,财务科和医保科负责收费的实时监控。

3.成立科室实施小组开展临床路径的相关科室必须成立科室实施小组。

科主任是第一责任人,实施小组成员对临床路径知识及相关内容进行认真学习,有计划地组织科室全体医护人员培训,实施临床路径管理,履行临床路径的资料管理、修订调整等职责。

4.工作职责(1)临床路径管理领导小组职责:①制定临床路径实施方案并组织实施。

②明确各有关部门的职责并负责组织协调各部门工作。

③确定实施临床路径管理病种与标准。

④组织人员培训。

⑤督查工作进展情况,实施临床路径的效益评价。

⑥研究解决实施中存在的问题,改进工作方法,促进医疗质量的全面提高。

(2)开展临床路径的相关科室科主任职责:①参与修订临床路径中与医疗相关的措施。

②决定病人是否进入或退出临床路径,并填写在临床路径记录本中。

③临床路径表内治疗项目的确定、计划和执行。

④对病人的康复进行评估,是否合乎临床路径的预期目标。

⑤定期阅读变异分析报告,提议讨论需要改良服务的项目。

(3)药剂科的职责:①监测合理用药。

②在保证质量的基础上,降低用药成本。

③协助处理与药物有关的变异。

(4)临床辅助科室(检验科、放射科、超声科、心电图室)职责:①执行临床路径表上本科室执行的项目。

②协助处理与本科室有关的变异。

五、临床路径的运行步骤(一)确定标准化医嘱标准化医嘱,是指依据某一病种的病情发展与变化,制定出该病种基本的、必要的、常规的医嘱,如治疗、用药、检查等等。

标准化的医嘱与临床路径的内容相对应,使之相对全面化、程序化、并相对固定,方便临床路径的顺序进行。

(二)培训对相关科室人员进行系统培训,使医、护、药、技人员明确各自职责。

(三)试行通过临床试行对临床路径进行检测,发现存在的问题,加以改进,逐步完善。

六、监管和评价科室实施小组每月常规统计病种评价相关指标的数据,并上报临床路径管理评价专家小组。

管理评价专家小组每季度对临床路径实施的过程和效果进行评价、分析并提出质量改进建议。

定期考核住院患者总人数、进入路径的患者总人次数、完成路径的人次数、出现变异的患者数、平均住院天数、平均药品费用、平均住院总费用、治疗有效率、病人满意度等指标,进行路径管理效果评价。

1、临床路径实施的过程评价内容包括:相关制度的制订、临床路径实施的记录、患者退出临床路径的记录等。

2、手术患者的临床路径实施效果评价应当包括以下内容:预防性抗菌药物应用的类型、预防性抗菌药物应用的天数、手术后并发症、住院天数、手术前住院天数、住院费用、药品费用、医疗耗材费用、患者转归情况、健康教育知晓情况、患者满意度等。

3、非手术患者的临床路径实施效果评价应当包括以下内容:病情严重程度、主要药物选择、并发症发生情况、住院天数、住院费用、药品费用、医疗耗材费用、患者转归情况、健康教育知晓情况、患者满意度等。

七、考核临床路径考评结果纳入医院绩效考核,严格奖惩兑现。

根据临床路径指导评价小组的评价结果,对临床路径开展好的科室及个人给予奖励。

白银市第二人民医院单病种质量管理工作实施方案为了进一步强化医疗管理,规范诊疗行为,持续改进和提高医疗质量,有效控制医疗费用,切实解决群众看病贵问题,根据卫生部、省卫生厅及市卫生局有关要求,结合我院实际,决定在全院开展部分单病种疾病诊治质量的控制工作。

特制定《白银市第二人民医院单病种质量控制工作实施方案(试行)》。
