Conducting The Ergonomic Workstation Self-evaluation
Step 1: Department Management conducts the initial self evaluation with new hire
or employee who transferred to a new workstation. This function may be delegated to another staff member, such as the supervisor or ergonomic advocate. The review is conducted using the self-evaluation handouts.
Step 5: Retain a copy of the self-evaluation and documentation of the changes in the
employee’s department file. Forward a copy of the form to the ergonomics office located in Human Resources.
