进展期胃癌的分型及CT表现

【摘要】目的探讨进展期胃癌的MSCT表现，旨在提高对该病的认识。方法回顾性分析经手术病理证实的48例进展期胃癌的MSCT表现。所有病例均行40排西门子螺旋CT平扫及2期动态增强扫描。结果48例进展期胃癌中BorrmannⅠ型4例，BorrmannⅡ型癌12例，Ⅲ型癌21例，Ⅳ型11例，均可见胃壁增厚，Ⅰ型呈息肉样增厚，Ⅱ型和Ⅲ型均有中央溃疡形成，周边粘膜隆起呈堤状，Ⅱ型堤壁角≦90o，Ⅲ型癌堤壁角>90o，Ⅳ型胃壁弥漫性增厚，胃腔变小。结论MSCT能很好显示进展期胃癌特征，并能明确诊断。%Objective To improve the recognition of advanced gastric cancer by analyzing the CT manifestations of advanced gastric cancer. Methods Retrospective analysis of the operation of 48 histologically confirmed cases of advanced gastric cancer MSCT performance. All cases were treated with 40 rows of Siemens spiral CT scan and 2 phase dynamic enhanced scan. Results There are 4 BorrmannⅠ,12 Borrmann II, 21 BorrmannⅢ and 11 Borrmann Ⅳ cancer cases in 48 cases of advanced gastric cancer. All of them have thickened gastric wal .BorrmannⅠshows polypoid thickening . Borrmann IIand BorrmannⅢ have central ulcer formation and the surrounding mucosa uplift in levee.
