耿连岐主任医师学术思想探源

耿连岐主任医师研读《黄帝内经》《伤寒论》,沉潜于斯二十余载,结合多年临证经验,潜心研究,在阴阳的层面上考虑疾病的本质病机,根于病机立法,本文将耿连岐主任医师的治病理念总结为“阳气为先,阴血为本;因势利导,辟除阴邪;五方平衡,六部通畅;阴平阳秘,气血乃和”。其中,“阳气为先,阴血为本”为治病的主导思想;“五方平衡,六部通畅”为脏腑六经辩证的理论指导;“因势利导,辟除阴邪”为根据疾病的趋势采用适宜的治疗手段,因势利导,最终达到“阴平阳秘,气血乃合”的健康状态。以下就耿连岐主任医师的学术思想,展开讨论,不揣鄙陋,一并和盘托出,以飨同道。

1阳气为先,阴血为本,治病不离阴阳之理《黄帝内经》作为我国现存最早的医学典籍,十分重视阳气对人体的影响。《素问·生气通天论》曰:“阳气者,若天与日,失其所则折寿而不彰,故天运当以日光明,是故阳因而上,卫外者也。”“阳气者,精则养神,柔则养筋。”说明阳气在人生中何其重要,比作天与日,能养精养神,主宰人的命运。扶持和保护阳气当是治疗及预防疾病的根本精神,阳气是机体生命活动的原动力,人体的阳气存之则生,失之则死[1-2]。正如《扁鹊心书》[3]载“为医者,要知保扶阳气为本。”因此,固护

一身之阳气,为诸法之先,耿主任由此提出阳气为先。《内经》中多处条文提到阳气和阴血的关系,如:“阴在内,阳之守也,阳在外,阴之使也”“阳化气,阴成形”。明代著名医家张景岳提到“阳动而散,故化气,阴静而凝,故成形”。最典型的阳和阴的关系是丈夫和妻子的关系,在“男主外,女主内”的古代社会,妻子为丈夫在家打点家务,丈夫为妻子外出工作,正是“阴在内,阳之守也,阳在外,阴之使也”的生动表现。对于人体而言,身体犹如家庭,阳和阴是家庭的丈夫和妻子。人体的正气是动态不断分化的,属阳;精血津液处于凝聚态,属阴。“阳化气”是说阳气可以把机体的物质

[摘要]天津市滨海新区名中医耿连岐主任医师的学术思想主要为“阳气为先,阴血为本;因势利导,辟除阴邪;五方平衡,六部通畅;阴平阳秘,气血乃和”。本文以阐释学术思想为核心,从阴阳理论、五行学说、天人相应理论展开讨论,将来源于古典医籍的耿连岐主任医师学术思想重新回归到中医经典著作中进行解读,提出治病首辨阳气的重要性,把扶助阳气作为治疗疾病的首选;把脏腑辨证和经络辨证有机的统一起来,治疗大法最后都统一到气血表里阴阳的统一中,最终达到中和的状态。

[Abstract]The academic thoughts of chief physician Geng Lianqi,a famous traditional Chinese medicine doctor in Binhai New Area of Tianjin,are mainly as follows:"Yang-qi is the first,Yin blood is the foundation;according to the situation,the evil of Yin is eliminated;five sides are balanced,six parts are unobstructed;Yin and Yang are constipat⁃ed,Qi and Blood are harmonious".This paper takes explaining the academic thought as the core,discusses from the Yin-Yang theory,the Five Elements theory and the theory of correspondence between heaven and man,reinterprets the academic thought of chief physician Geng Lianqi,who originated from the classical medical books,into the classical works of traditional Chinese medicine,and puts forward the importance of differentiating Yang-qi first in treating dis⁃eases,and regards supporting Yang-qi as the first choice for treating diseases,and combining the viscera syndrome dif⁃ferentiation and meridian syndrome differentiation organically,and the treatment methods are finally unified into the u⁃nification of Qi and Blood,Yin and Yang,the outside and the inside,and finally reach the state of neutralization.
