我司要求上述提单作如下修改(We request to amend B/L as follows):

我司无条件承担因上述更改提单内容而产生的一切责任、风险和损失(We are fully liable for all risk, damage or loss caused by the amendment)

对于涉及货物品名的更改(Regarding Amendment on Commodity Name)

我司保证更改后品名与报关资料完全一致，有效报关联正本附送贵司核实，并自行负责向海关申请清洁舱单之更改。我司愿意承担由此产生的任何责任及费用(We hereby guarantee above new commodity name is completely consistent with Customs declaration. The valid Original Customs Declaration Sheet is presented for verification as well. We take full responsibility for correcting Clean Manifest with Customs on our own account/risk. We here to undertake all responsibility & cost that may incur.)

注意事项(Notes)：

1．正本保函需一式两份(LOI in duplicate)

2．本保函内容必须用打字机打印，涂改无效(Typewriter print, no handwriting, no alteration)

3．只需打上有更改之项目内容(Content for items to be amended only)

4．每份保函只得对应一份提单(One LOI for one B/L only)
