三叉神经痛有关的疼痛性疾病研究中，均发现有多种神经肽

参与其中，与疼痛有关的包括谷氨酸（glutamic acid ，Glu ）、降钙素基因相关肽（calcitonin gene related protein ，CGRP ）、血管活性肠肽（vasoactive intestinal peptide ，VIP ）、P 物质（substance P ，SP ）、生长抑素（somatostatin ，SS ）、β⁃内啡肽（β⁃endorphin ，β⁃EP ）等，它们都与原发性三叉神经痛关系密切。笔者对神经肽与原发性三叉神经痛的关系做一综述。1　CGRP

1.1　CGRP 的生物学作用　CGRP 在三叉神经节神经元的胞体合成后，以快速轴浆运输的形式，分别运输至外周端及中枢端，在此发挥作用。根据神经生物学研究显示，CGRP 在口腔颌面部的作用很多，主要有以下几个方面：（1）传导痛觉等伤害性信息：CGRP 是初级感觉神经元的兴奋性递质，顺行向中枢传导痛觉等伤害性信息，而逆行释放于局部组织中可引起痛觉过敏；（2）有研究显示，咬合创伤可引起

牙髓及牙周组织中CGRP 阳性纤维增多〔1⁃2〕

。由于CGRP

作为伤害性刺激的递质或调质存在于感觉神经元中，因此，

在有关疼痛的研究中引起人们的高度重视。

1.2　CGRP 的致痛机制　CGRP 介导的机械性伤害刺激传递是借助促进脊髓背角释放SP 以促进痛觉的传递实现的，CGRP 阳性神经元主要位于脊髓背根节和三叉神经节内，具有强烈的扩血管作用，并能加强速激肽增加毛细血管通透性

血浆蛋白渗出的作用。CGRP 可与多种神经递质共存，甚至共存于一个分泌颗粒内。脊髓珠网膜下腔注射CGRP 可使痛域降低，注射CGRP 抗体可提高痛域。CGRP 和SP 共存，

提示CGRP 可促使SP 的释放而利于痛信号的传递〔3〕

。Goadsby 等〔4〕用电刺激5只猫的三叉神经节后，产生类似的

三叉神经痛，发现颈外静脉血中SP 、CGRP 含量显著升高。通过实验证实，原发性三叉神经痛发作时，颈外静脉的SP 、CGRP 含量确实高于术后缓解期，痛支神经纤维中CGRP 免疫反应阳性颗粒的数量、面积等显著多于非痛支神经。认为

三叉神经痛发作时，痛支神经过度合成和释放CGRP ，促进

SP 的大量产生，导致阵发性剧痛〔5⁃7〕。随着SP 的耗竭，疼

痛消失。陈志峰等〔8〕通过对大鼠的研究表明，用铬线疏松结

扎大鼠右侧眶下神经，大鼠于术后9d 起持续出现机械性痛觉过敏，是由于CGRP 等神经肽类的突触前、后效应，提高延髓内兴奋性氨基酸作用，通过NMDA 受体上的作用，引起神经元兴奋性增强，这种兴奋性增加可受CGRP 等神经肽的释放而易化，其结果是感受野的扩大，并导致程度更强、时间更

长的疼痛〔9〕。进一步证实了CGRP 在三叉神经痛发作中的
