多发性骨髓瘤(multiple myeloma ,MM )是血液系统常见的恶性肿瘤之一,约占肿瘤的1%,血液肿瘤的
10%,是以骨髓中浆细胞异常增殖为特征,骨髓瘤细胞产生并分泌单克隆免疫球蛋白(M 蛋白)而损害组织细胞的恶性肿瘤[1-2]。MM 的诊断和疗效标准主要由国际骨髓瘤工作组(international myeloma working
group ,IMWG )制定,分期标准参照国际标准分期(intern-ational staging system ,ISS ),主要依赖血清β2微球蛋白
(β2microglobulin ,β2-MG )、血清白蛋白(albumin ,ALB )、血清钙(Ca )、血肌酐(Cr )、血红蛋白(Hb )、红细
胞分布宽度(RDW )及乳酸脱氢酶(LDH )等指标评估肿瘤负荷及疾病的预后[3-4]。由于骨髓瘤细胞的异质性,患者的生存期差异很大,近些年,虽有新型药物的不断问世,使MM 患者生存期明显延长,但仍不能完全治愈[5]。红细胞分布宽度(erythrocyte distribution width ,RDW )一般用于贫血的鉴别诊断,但近年来,有文献报道RDW 在炎症中存在一定的诊断意义,且在实体瘤及部分血液病中亦有研究,但在MM 患者中报道较少[6]。本文收集了65例MM 患者的临床资料,分析RDW 与MM 预后因素之间的关系,探索RDW 在MM 预后中的意义。基金项目:甘肃省人民医院院内科研基金项目(项目编号:17GSSY2-2)第一作者:马婷,女,主治医师,从事血液系统常见疾病诊疗工作

【摘要】目的:探索红细胞分布宽度(RDW )在多发性骨髓瘤(MM )患者中的临床价值。方法:收集2011年1月至2018年1月在甘肃省人民医院血液科就诊的初发65例MM 患者作为病例组,83例同期健康体检者为对照组,比较两组的RDW ,绘制受试者工作曲线(ROC 曲线),取RDW 为13.55%作为最佳截点,将65例MM 患者分为低RDW 组(RDW ≤13.55%)和高RDW 组(RDW >13.55%),分析两组患者在年龄、性别、血清钙(Ca )、血清β2微球蛋白(β2-MG )、白蛋白(ALB )、血肌酐(Cr )、血红蛋白(Hb )、RDW 及乳酸脱氢酶(LDH )的差异;比较两组患者生存率的差异,Cox 多因素分析其与MM 预后的关系,并对高RDW 组与低RDW 组化疗前后RDW 进行比较。结果:MM 患者RDW 水平明显高于对照组(P <0.05);高RDW 和低RDW 在年龄、性别、Ca 、β2-MG 、ALB 、Cr 、Hb 、N 、L 、NLR 、RDW 及LDH 的比较无统计学差异(P >0.05);与低RDW 组比较,高RDW 组化疗后RDW 值降得更快;Cox 多因素分析显示,N 、RDW 与生存时间有关(P <0.05)。结论:RDW 是初诊MM 患者预后的危险因素,RDW 升高提示患者预后较差。

【Abstract 】Objective :To study the clinical value of erythrocyte distribution width (RDW )in multiple myeloma (MM ).Methods:
Sixty-five patients from January 2011to January 2018in hematology department of Gansu provincial hospital were collected as case group.
Sixty-five patients with initial MM and 83healthy subjects at the same time were included in the study.RDW was obtained according to
blood routine results and the differences between the two groups were compared.ROC curves were drawn and took RDW 13.55%as the best cutoff point ,the 65MM patients can be divided into low RDW group (RDW ≤13.55%)and high RDW group (RDW>13.55%),the analysis of two groups of patients in age ,sex ,serum calcium (Ca ),serum beta 2microglobulin (β2-MG ),albumin (ALB ),serum creatinine (Cr ),
hemoglobin (Hb ),red blood cell distribution width (RDW )and lactate dehydrogenase (LDH )differences ;The difference in survival rate
between the two groups was compared ,and Cox multivariate analysis was performed to analyze the relationship between the above indicators
and MM prognosis ,The RDW of the high RDW group was compared with the low RDW group before and after chemotherapy.Results:The level of RDW in MM patients was significantly higher than that in healthy control group (P <0.05).There was no significant difference between high RDW and low RDW in age ,gender ,Ca ,β2-MG ,ALB ,Cr ,Hb ,N ,L ,NLR ,RDW and pared with low RDW group ,RDW in high
RDW group decreased faster after chemotherapy ;The results of Cox multivariate analysis showed that N and RDW were related to survival time (P <0.05).Conclusion :RDW is a risk factor for prognosis of newly diagnosed MM patients.High RDW suggests a poor prognosis.
