患者对于在陌生人面前或可能被别人仔细观察的社交或表演场合，有一种显著且持久的恐惧，害怕自己的行为或紧张的表现会引起羞辱或难堪。
有些患者对参加聚会、打电话、到商店购物、或询问权威人士都感到困难。
在心理学上被诊断为社交焦虑失协症（SAD）。
基本概念社交恐惧症俗称“见人恐怖症”，是恐怖症中最常见的一种，约占恐怖症病人的一半左右。
社交恐惧症是一种对任何社交或公开场合感到强烈恐惧或忧虑的精神疾病。
患者对于在陌生人面前或可能被别人仔细观察的社交或表演场合，有一种显著且持久的恐惧，害怕自己的行为或紧张的表现会引起羞辱或难堪。

在心理学上被诊断为社交焦虑失协症（soci al anxiety disorder , SAD），是焦虑症的一种。
此症最早起源于西元1985年，当时被认为是忽略性焦虑失协症（Neglect anxiety disorder），经过14年后才渐渐被重视。
相关分析一般人对参加聚会或其他会暴露在公共场合的事情都会感到轻微紧张，但这并不会影响到他们出席。
真正的社交恐惧症会导致无法承受的恐惧，严重的案例里，病患甚至会长时间的把自己关在家里孤立自己。
社交恐惧症患者总是处于焦虑状态。
他们害怕自己在别人面前出洋相，害怕被别人观察。
与人交往，甚至在公共场所出现，对他们来说都是一件极其恐惧的任务。
社交恐惧症不应该与恐慌症（panic disorder）混淆，恐慌症患者相信他们的恐慌是由某些严重的物理原因造成，在发作当时或之后往往去医院或叫救护车。
社交恐惧症患者也许会经历恐慌发作，但是他们会察觉到自己经历的是由非理性的恐惧造成的极大焦虑。
很少社交恐惧症患者愿意在那时去医院，因为他们害怕权威人士的拒绝或评断。
与权威人士打交道对大部份社交恐惧症患者来说特别困难，像是打电话询问、参加约会、派对、或工作面试等等。

符合神经症的诊断标准 以恐惧症状为临床相，符合以一各项：
对某些客体或处境有强烈恐惧，恐惧的程度与实际危险不相符。 发作时有焦虑和植物神经症状。 有反复或持续的回避行为。 知道恐惧过分或不必要，但无法控制。
对恐惧情境或事物的回避必须是或曾经是突出的症状。 排除焦虑症、疑病症和精神分裂症。

收到一封19岁的女孩燕儿寄来的信，信中以痛苦又无奈 的口吻向心理医生诉说了她的烦恼，急切地等待着心理医 生的帮助。 来信的大致内容是这样的：“我从小胆子就小，最怕的就 是蛇，以前只是看见它才害怕，但是现在我只要看见它的 图片或蛇年的邮票，说话时提到“蛇”这个字或“蛇”的英文 单词snake，我都会做出强烈的反应，高声尖叫或者大声 地哭。我第一次见到蛇，只是大声的尖叫，然后就没事了。 可是现在，我的脑子里全是蛇，甚至连眼睛都不敢闭上， 我害怕得要发疯，心里难受得要命。我不能自拔，同学也 说我整天吓得她们不知所措，她们说我不可思议，竟被吓 成那样。我知道这是心理问题，但我却无法控制。尊敬的 心理医生，我该怎么办？” 看了燕儿的信，我想起弗洛伊德的一句话：上帝之所以悲 天悯人是因为人类的心灵太脆弱了。相信所有读者都会和 我一样对燕儿的境遇深感同情，作为心理医生，我希望能 帮她走出困境，重新找回恬静的心灵。经过分析，我认为 她患了动物恐怖症。
恐怖症 Phobia,phobic neurosis
恐怖症 Phobia,phobic neurosis恐怖症（Phobia,phobic neurosis）是以恐怖症状为主要临床相的一种神经症。
患者对某些特定的对象产生强烈和不必要的恐惧，伴有回避行为。
恐惧的对象可能是单一的或多种的，如动物、广场、闭室、登高或社交活动等。
患者明知其反应不合理，却难以控制而反复出现。
青年期与老年期发病者居多，女性更多见。
国外报道一般人口中的患病率为77‰，我国各地调查患病率的平均值为2‰左右。
[病因与发病机理]病因未明，可能与下列因素有关：一、遗传因素：Slater等（1977）报道患者的一级亲属中，20%的父母和10%的同胞患神经症，认为遗传因素可能与发病有关。
也有人指出：至今尚无证据表明遗传在本病的发生中起重要作用。
二、性格特征：病前性格偏向于幼稚、胆小、含羞、依赖性强和内向。
三、精神因素：在发病中常起着更为重要的作用。
例如某人遇到车祸，就对乘车产生恐惧。
可能是在焦虑的背景上恰巧出现了某一情境，或在某一情景中发生急性焦虑而对之发生恐惧，并固定下来成为恐怖对象。
对特殊物体的恐怖可能与父母的教育、环境的影响及亲身经历（如被狗咬过而怕狗）等有关。
心理动力学派认为恐怖是被压抑的潜意识的焦虑的象征作用和取代作用的结果。
条件反射和学习机理在本症发生中的作用是较有说服力的解释。
[临床表现]恐怖症通常急性起病，以面对某一物体或处境爆发一次焦虑发作为先驱，患者虽知这种恐怖是过份和不必要的，但不能克制，不接触或脱离所恐怖对象时，则表现正常，因此，常伴有回避行为。
恐怖对象可归纳为三类：一、处境恐怖：对街道、广场、公共场所、高处或密室等处境恐惧，因此不敢出门，而回避这些场所。
二、社交恐怖：对需要与人交往的处境感到恐怖而力求避免，如与人交谈等。
三、单纯恐怖：如对针、剪、刀、笔尖等物体发生恐怖时称锐器恐怖；对猫、狗、鼠、蛇等动物发生恐怖称动物恐怖。
[病程和预后]病程虽长，但预后良好。
一般经数年后可逐渐好转或痊愈。

恐惧症及其护理恐惧症(phobia)也称恐惧性神经症((phobia neurosis) ，恐怖症，恐惧性焦虑障碍。
是一种以过分和不合理地惧怕外界客体、处境或与人际交往为主要特征的神经症。
患者明知恐惧没有必要，但仍不能防止其发生，恐惧发作时常伴有明显的焦虑和自主神经症状。
患者极力回避所害怕的客体、处境或人际交往，或是带有畏惧去忍受，因而影响其正常生活、工作、学习与社会交往。
常见的临床类型有三种：场所恐惧症、社交恐惧症和特定恐惧症。
恐惧症的特点1、恐惧的对象存在于客观环境中；2、焦虑、恐惧情绪是指向特定的对象的；3、焦虑、恐惧的程度与现实威胁不相符合；4、回避是缓解焦虑、恐惧的主要方式；5、患者能够认识到恐惧的不合理性，但又不能控制。
一、病因及发病机制（一）心理社会因素1、心理素质因素：部分患者具有内向、胆小、害羞、被动、依赖、焦虑等人格特点。
幼年受到母亲过度保护，成年后易患此症。
2、精神动力学派：强调患者童年的经历，如羞辱或批评性遭遇、父母的不和、童年丧失父母或与父母分离等。
在以后生活事件的诱发下，易出现焦虑与恐惧。
3、学习理论的观点：行为主义学派强调条件性学习在恐惧症发病中的作用，认为恐惧症是患者通过条件学习和自我强化而固定下来的习惯性行为。
4、认知理论的观点：认为恐惧症是患者高估所害怕情景或事物的危险性所致。
对所面临的对象或处境给与夸大或歪曲的认知，从而导致恐惧情绪的出现。
（二）生物学因素1、遗传因素：有证据证明特定恐惧症具有家族聚集性，遗传因素可能起一定作用。
通过双生子同病率和家系的研究提示，遗传因素在社交恐惧症的发病具有中等程度的作用，但在场所恐惧症中的作用并无定论。
2、神经生化：研究提示，NE、5-HT、GABA系统以及下丘脑-垂体-肾上腺轴有可能参与恐惧症的发病。
3、神经影像：研究提示，社交恐惧症可能与基底结和纹状体的DA功能障碍有关。
已有研究显示，社交恐惧症患者比正常人对照在纹状体内的DA回吸收位点密度减低。
