摘要目的探讨血清激光拉曼光谱鉴别肺癌、慢性阻塞性肺疾病(COPD)患者及健康人的价值。方法收集16例肺癌患者(肺癌组)、13例COPD患者(COPD 组)和17例健康人(健康对照组)的外周静脉血血清,用Labram HR 800激光共聚焦拉曼光谱仪检测,每个血清样品测5个点,将同一样品的5条光谱平均,以此代表该样品的血清拉曼光谱信号,将3组的拉曼光谱信号数据采用单因素方差分析、主成分分析(PLA)、线性判别分析(LDA)、交叉检验及受试者工作特征曲线(ROC)下面积等进行统计学处理。结果肺癌和COPD组拉曼波谱较健康组蓝移,在1000cm-1处以肺癌组蓝移最明显,P<0.05。肺癌和COPD组在大多数拉曼谱强均弱于健康对照组,在825cm-1、848cm-1、1202cm-1及1266cm-1处COPD组最弱,P<0.05。肺癌和COPD组拉曼谱强在1083cm-1、1103cm-1处强于健康对照组,P<0.05。统计学处理后的数据表明,激光拉曼光谱技术对肺癌及COPD的判别灵敏度为分别为87.50%(14/16)和76.92%(10/13),健康人的判别特异度为100%(17/17),判别准确性为89.13%(41/46)。肺癌-健康人、COPD-肺癌及COPD-健康人ROC 曲线下面积分别为:0.967、0.827和0.927。结论血清拉曼光谱分析可有效鉴别肺癌、慢性阻塞性肺疾病及健康人,可以为肺癌的早期、无创诊断提供新的检测方法。

Abstract Objective To discuss the value of serum laser Raman spectroscopy in discriminating of patients with lung cancer, patients with chronic obstructive pulmonary disease(COPD) and healthy people. Methods Test ed serum of peripheral venous blood from 16 patients with lung cancer (lung cancer group), 13 patients with COPD (COPD group) and 17 healthy volunteers (healthy control group) with Labram HR 800 confocal Raman spectroscopy .Each serum sample was measured five spots, the average of 5 spectra in 1 sample represented the signal of Raman spectra in one case .The Raman spectrum signal data of the three groups were statistically analyzed by ANOVA analysis, principal component

analysis(PLA), linear discriminant analysis(LDA),cross-validation and area under the receiver operating characteristic curve (ROC).Results Raman band near 1000cm-1of lung cancer group and COPD group shifted bluer than healthy group , lung cancer group shifted most among three groups, P <0.05. Most band of Raman intensity of lung cancer and COPD groups were weaker than that of healthy group, the weakest one was the COPD group at band of 825cm-1、848cm-1、1202cm-1 and 1266cm-1, P <0.05. At band of 1083cm-1 and 1103cm-1Raman intensity of lung cancer and COPD groups were stronger than that of healthy group, P <0.05. The discrimination showed

s ensitivities for lung cancer and COPD were 87.50% (14/16) and 76.92% (10/13), specificity for healthy people was 100% (17/17), accuracy was 89.13% (41/46). Area under the ROC curves of Lung cancer - healthy people, COPD-lung cancer and COPD-healthy people were 0.967,0.827 and 0.927, respectively.Conclusion Technology of laser Raman spectroscopy can effectively discriminate the serum of lung cancer, chronic obstructive pulmonary disease and healthy people.It is expected to provide early noninvasive diagnosis of lung cancer as new detection methods.
