美国住院医师制度(专科医师制度)——有感于国内的住院医师培训制度,整理分享了下老美的培训,好的吧,和他们相比,我们还不是被整的最惨的。

美国,报考医学院的考生一般要完成4年理工科大学本科的学习(以前主要是理工科,现在也逐步增加人文学科的比例),其中必须完成相应课程的学习,然后通过MCAT考试,进入医学院校就读,学制4年,前两年基础,后两年临床。

目前全美共有医学院校120多所,年招生总规模控制在16000名左右。

学生在大学二年级结束时需参加全美医师资格考试的第一步考试(step 1),通过测试后方可进入高年级的学习,大学毕业前进行第二步考试(step 2)。

通过前两步考试后,学生可根据自己的兴趣爱好并结合自己的学习成绩报名各大学医学院附属医院或医学中心的相关学科进行住院医师培训。

最后一步资格考试(step 3)需在住院医师训练结束时进行。

只有通过以上三步全美医师资格考试,才能有资格成为主治医师(也就是实际意义上的医师),并申请各自感兴趣的医院从事独立的临床工作。

住院医师培训是毕业后医学教育的一个重要组成部分,在这个阶段,根据培训计划,住院医师在上级医师的指导下进行临床实践,通过规范的要求和严格的考核,从而学习到某一认可的医学专科领域中合格医生所必需的知识、技能和态度。

在美国,住院医师培训地点多选择在有条件的大医院中进行,全美有1700所医院接受住院医师培训。

住院医师培训计划,每年5月由毕业后医学教育联合委员会召集会议,并由医学教育委员会认可,决定下年度全国住院医师培养计划,汇编成住院医师培养计划指南(Directory of Residency training Programs),向全美以及国外医学院毕业生提供训练职位。

培训职位的选择根据毕业生的志愿采取自愿申请制度,然后医院面试决定是否给予职位,这步叫MATCH PROGRAM。

为确定首选专业和第二选择专业,每一名毕业生平均要向26家医院申请。

对首选专业,平均要接受10家医院的面试,从提出申请到得到面试通知的概率为63%,各家医院对首选专业拒绝给予面试的概率为20%,其范围根据各专业的竞争程度而有所不同。

当我真正成为一名家庭医生后,我对于全科医学这一学科有了新的认识,对于家庭医生这一职业有了更充分的了解。

家庭医生是全科医疗的提供者。

家庭医生与专科医生有着不同的职责。

专科医生负责疾病形成之后的诊治,通过深入的研究来认识与对抗疾病。

而家庭医生则是负责健康时期、疾病早期乃至经专科诊疗后无法治愈的各种病患的长期照顾,其关注的中心是人。

家庭医生与专科医生是各司其职、相辅相成的。

一名合格的家庭医生不但可以帮助专科医生提高医疗服务效率,而且还能更好地控制疾病的发展,提高患者的生存质量。

那么如何成为一名合格的家庭医生呢?我认为一名合格的家庭医生应具备以下几项素质。

首先,当然是要具备综合性的知识。

在国外,家庭医生是社区卫生服务体系中必不可少的。

家庭医生要根据病人的病情进行分配,是需要到大型医院就诊还是在社区卫生服务中心进行治疗。

家庭医生在病人就诊过程中扮演着重要的角色,这就对于家庭医生的知识综合性有很高的要求。

在美国,要想成为一名合格的家庭医生需要经过三阶段的培训:第一阶段,5年制的医学院校本科学习;第二阶段,住院医师培训;第三阶段,家庭医生的继续医学教育培训。

在这种制度培养的制度下,保证了家庭医生拥有丰富的专业知识和临床经验,更好的对于疾病进行分类和初步诊断。

尽管,这种培训模式在中国实施起来不太容易,但作为一名家庭医生,要时刻注意医学相关知识,不断提高完善自己的技能。

家庭医生面对着的是一个区域长期固定的人群,家庭医生的职责中不仅有治疗疾病还有预防疾病和控制疾病的发展。

家庭医生面对的疾病多以慢性病为主,需要长期对患者进行医学干预,才能更好的控制疾病。

事后和科室的主任谈起来,主任告诉我,他也要求医生们读书,可是他们不肯读,当主任的也没有办法,只要他们工作不出事,主任无权处理这些不学无术的医生。

这位主任谈起这个问题时,显得非常的无奈。

同时主任得常常盯着这些医生的工作,以防这些医生出事,这样一来,主任感到压力很大。

在这样的制度下,住院医生没有太大的压力去提高自己的水平。

美国的住院医生工作非常繁重,精神压力很大,工作时间很长(80-100小时/星期)。

除了应付日常工作,每个住院医生还有阅读任务及参加科室考试。

住院医生完成培训后要参加全国统一的考试来获取其专科文凭,这个考试大多数是笔试,就是考理论,有的专科还有口试。

而且考试题目中相当一部分题目涉及到一些严重威胁病人的生命,但又少见或罕见(high riskand lowfre-quency)的疾病,例如下文所提的恶性高热症。

既然罕见,在培训过程中就不一定能碰到。

大家只能从书上学习前人的经验,这种病例如果不能妥善处理,病人就会死在医生手里。

医生们不读书根本无法考试及格。

在培训中表现不好的住院医生将会受到处分甚至被迫离开。

住院医生只是在培训医院接受培训,培训完成后他们再到全国各地找工作。

在找工作的过程中,主任的介绍信是最重要的。

相对于国内医院培训年轻医生的如履薄冰,美国住院医师培训是一个以循证医学为中心,高度标准化的过程,通过规范的要求和严格的考核,从而学习到某一医学专科领域中合格医生所必需的知识、技能和态度。

1.美国住院医工资待遇在美国,Intern指第一年住院医;Resident指第二、三年住院医;Fellow为住院医与主治医之间的培训阶段,一般为3年,相对于主治医生,住院医的工资待遇会相对低些,住院医(包括fellow)的工资一般在4500USD/月左右,一年有5-6万USD。

But I know I have to be ready to leave for school by 8:30am. My baby sister Sophie is the last one up at8am. My mom gets her dressed, brushes her hair, and they play games together until it's time to take her to preschool at 9am.During the day, my mom and dad are at work. My mom is a nurse at the hospital downtown. Every day, she takes care of sick patients by giving them medicine, changing bandages, and making them feel comfortable. Sometimes she works the day shift from 7am to 3pm. Other days, she works the night shift from 7pm to 7am. It's a very hard job!My dad spends his days downtown too, working at a big law firm. He has to read through lots of papers, meet with clients, go to court hearings, and do research on cases he's working on. His daily routine is pretty busy from 9am to 6pm.After school from 3:30pm to 6pm, either my mom or dad will pick me and Michael up if they're not working late. We'll hang out at home, have snacks, play outside if it's nice out, and work on our homework. Around 6pm, my dad gets home from his job and my mom starts cooking dinner if she's on the day shift that day.Most nights after 6:30pm, our family routine is to eat the dinner my mom made together and talk about our days. Michael and I clean up the dishes and clear the table when we're done eating. Then we all watch a TV show or movie together until 8pm.At 8pm, it's time for my bedtime routine. I go upstairs, take a bath, put on my pajamas, and read books for 30 minutes. My mom gives me a hug and tucks me into bed at 8:30pm. She'll check on me one more time at 9pm to make sure I'm sleeping soundly.Michael's bedtime is a little later at 9pm. He takes a shower, reads, and gets in bed by 9:30pm. As for my parents, they'll stay up doing chores like laundry or just relaxing until around 11pm. If my mom is working the night shift at the hospital, she has to be there by 7pm. So instead of a bedtime, she gets ready and leaves for her job in the evenings those nights.On the weekends, our routines are more relaxed as a family. We'll typically do fun activities together like going to the park, seeing a movie, getting brunch, or running errands. My favorite is when we go hiking or camping! We also spend time visiting my grandparents and other relatives.One routine we have no matter what day it is - we all have to go visit the doctor once a year for our annual checkups. Our doctor is named Dr. Martin and he works at a big health clinic near our home. Whenever I get a fever, the flu, or any other sickness, we see him too. He's a really nice doctor who always makes sure we're healthy.Going to the doctor isn't my favorite thing in the world, but I know it's important. First, the nurse calls me back from the waiting room to check my weight and temperature. Then I get to go into one of the patient rooms. There, I wait a little while until finally Dr. Martin comes in.He always greets me with a huge smile and asks how I've been. Using his stethoscope, he'll listen to my heart and lungs. He checks my throat and ears too. If I need any vaccines or shots that year, he'll give them to me. I have to admit, I don't like getting shots at all! They pinch my arm and it stings. But Dr. Martin is great at calming me down and telling me I'm being so brave.After my checkup, the doctor prints out an after visit summary which reminds me when to come back next year. I get a lollipop or sticker too for being a good patient! My mom makes sure to schedule the next year's appointment before we leave.
