阿托伐他汀对新西兰兔胰岛素敏感性的急性影响
摘要:目的研究阿托伐他汀对新西兰兔胰岛素敏感性的急性影响。方法 40只健康的新西兰大耳白兔,随机分为正常对照组(NC组)、阿托伐他汀5mg组(A 组,5mg/kg•d),阿托伐他汀10mg组(B组,10mg/kg•d),阿托伐他汀20mg组(C组,20mg/kg•d),每组10只。喂养3天后,于第四天清晨空腹采血测定血糖及血清胰岛素浓度,计算出胰岛素敏感指数,并行静脉葡萄糖耐量试验(IVGTT),于不同时间点采血分离血清测定血糖、血清胰岛素水平。结果与NC组比较,阿托伐他汀组空腹血糖及空腹血清胰岛素水平显著升高(P<0.05),胰岛素敏感指数(ISI)则显著降低(A组、B组P<0.05,C组P<0.01);在IVGTT 中,A组、B组、C组的血糖下降迟缓,胰岛素分泌高峰较NC组降低(P均<0.05),达峰时间延迟。结论高剂量阿托伐他汀能够影响新西兰兔的糖代谢,降低胰岛素敏感性,且随阿托伐他汀剂量的增加,影响越明显。

Acute effects of atorvastatin on insulin sensitivity in New Zealand rabbits
Abstract: Objective To evaluate the acute effects of atorvastatin on insulin sensitivity in New Zealand rabbits. Methods Forty healthy New Zealand white rabbits were randomly assigned into 4 groups: normal control group(Group NC),atorvastatin 5mg group(Group A,5mg/kg•d),atorvastatin 10mg group(Group B,10mg/kg•d),and atorvastatin 20mg group(Group C,20mg/kg•d).Every group had 10 rabbits. Three days after feeding, on the fourth day morning, blood samples for fasting glucose and serum insulin measurements were withdrawn from auricular veins after fasting overnight. Insulin sensitivity index (ISI) was calculated. Glucose tolerance test was performed
on conscious rabbits following an overnight fast by intravenous administration of glucose (0.6g/kg).The blood samples for measurements of glucose and insulin were withdrawn at different time points. Results Compared with the normal control group, the levels of fasting blood glucose and serum insulin were significantly increased
(P<0.05), and the ISI was significantly reduced (Group A、Group B,P<0.05;Group C,P<0.01) in atorvastatin groups. In IVGTT, compared with the group NC, the blood glucose level of atorvastatin groups declined slowly, the peak of insulin secretion decreased (P<0.05), and the time to peak was delayed. Conclusion High-dose atorvastatin can affect glucose metabolism in New Zealand rabbits, reduce insulin sensitivity, and with the increase of dose of atorvastatin, the impacts are more apparent.

他汀类药物是羟甲基戊二酸单酰辅酶A(HMG-CoA)还原酶抑制剂,是临床最常用的一类降脂药物。他汀可以降低血浆总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)和甘油三酯(TG)水平,升高血浆高密度脂蛋白胆固醇(HDL-C)[1],具有抗动脉粥样硬化、改善血管内皮功能等作用。自从1987年美国FDA批准第一个他汀类药物-洛伐他汀上市以来,相继有多个他汀类药物广泛应用于临床[2]。然而,新近的一项荟萃分析显示,他汀类药物治疗使糖尿病的发生风险增加了9%[3]。本研究观察阿托伐他汀对新西兰白兔胰岛素敏感性的急性影响。
1 材料与方法
1.1实验材料
1.1.1实验动物 3月龄健康雄性新西兰大耳白兔40只,体重(1.8±0.2)kg,由厦门大学医学院实验动物中心提供,在厦门大学医学院实验动物中心饲养、观察研究。采用架式笼养,同室分笼饲养,自然光照,定时喂食,自由饮水,室内保持适合温度及湿度,集中通风。
1.1.2 试剂阿托伐他汀钙片(立普妥,辉瑞制药有限公司),50%葡萄糖注射液(湖南金健药业有限责任公司),胰岛素放射免疫分析药盒(北京北方生物技术研究所),葡萄糖氧化酶法试剂盒(德国Beckman公司)。
1.1.3仪器和设备 CX-7型全自动生化分析仪,德国Beckman公司;SN-695型智能放免γ-测量仪,上海原子核研究所日环仪器一厂产品;大容量低温离心机(放射免疫专用),上海安亭科学仪器厂产品;低温高速离心机,Eppendorf公司;超低温冷冻冰箱,日本三洋(SANYO)电器集团;Libror AEG-220电子天平,日
