Effect of Sodium Channel Blocker on Cortical Excitability in Patients with Focal Epilepsy

Abstract
Objective: To investigate the difference of excitability of cerebral cortex between patients with focal epilepsy and those with antiepileptic drugs (AEDs). Methods: The subjects were divided into five groups: focal epilepsy without medication (n= 14), focal epilepsy with medication (n= 17) and normal group (n = 15). The subjects who met the standard were selected to measure the rest-ing motor threshold, the amplitude and latency of motor evoked potential by transcranial mag-netic stimulation, and the measured values were compared statistically. Results: In the compari-son of resting motor thresholds among groups, there were statistically significant differences be-tween the focal epilepsy without medication and the group of focal epilepsy with medication (P =
0.017). The significant differences in the amplitude of motor evoked potential among groups were
found in the comparison of the focal epilepsy without medication and the normal control group respectively (P1 = 0.033). The same difference lies in the group of focal epilepsy with medication and the normal control group respectively (P2 = 0.038). Conclusion: The cortical excitability of pa-tients with focal epilepsy is significantly higher than that of normal people, and sodium channel blocker can significantly inhibit the cortical excitability level of patients with focal epilepsy.

要
目的:探讨局灶性癫痫患者及钠通道阻滞剂类抗癫痫药物(Anti-epileptic drugs ,AEDs)对受试者大脑皮层兴奋性影响的差异。方法:受试者分为局灶未服药组(n = 14)、局灶服药组(n = 17)及正常对照组(n = 15)共3组。利用经颅磁刺激技术测量受试者静息运动阈值、运动诱发电位波幅及潜伏期并对测量值进行统计比较。结果:局灶未服药组与局灶服药组间静息运动阈值的差异具有统计学意义(P = 0.017);运动诱发电位波幅的明显差异出现在局灶未服药组(P 1 = 0.033)以及局灶服药组(P 2 = 0.038)分别与正常对照组的比较中。结论:局灶性癫痫患者较正常人群皮层兴奋性水平明显升高;钠通道阻滞剂能够明显抑制局灶性癫痫患者皮层兴奋性水平。

1. 引言
癫痫作为神经系统最常见的疾病之一,在人群中的发病率高达0.7%~1.1%,我国有近千万癫痫患者,数量巨大。癫痫逐年升高的发病率以及近三分之一难治性癫痫患者的存在,使得其病理生理机制研究成为神经科的热点问题之一[1]。针对这一热点,国内外的学者采用各种方法进行了大量研究,但尚未有统一的观点。可以确认的是,由兴奋性和抑制性神经通路参与的潜在的过程是导致癫痫患者过度放电的基本环节,初级运动皮层是癫痫异常放电的关键部分[2]。在现有的研究中,经颅磁刺激(transcranial magnetic stimulation, TMS)是唯一可以主动对大脑功能进行干预并探究大脑高级功能的技术,通过对皮层兴奋性的评估,有助于理解癫痫等原发性中枢神经系统疾病的发病机制[3]。本研究将局灶性癫痫患者按照其服药情况进行分组后测定相关TMS 参数,通过对所测得的数据进行统计学分析,分析不同的条件下受试者的皮层兴奋性测定值是否具有统计学差异,更进一步探讨皮层兴奋性用于癫痫辅助诊断等方面的积极意义。
2. 材料与方法
2.1. 研究对象
所有实验组受试者均为2018-12-01至2019-12-31就诊于青岛大学附属医院癫痫专病门诊的局灶性癫痫患者,对照组受试者来源于青岛大学附属医院查体中心或志愿者。
