系膜增生性肾小球肾炎患儿Bax蛋白表达与外周血淋巴细胞凋亡的关系

【摘要】目的明确病理类型为系膜增生性肾小球肾炎(mesangial proliferative glomerulonephritis,MsPGN)的原发性肾病综合征(nephrotic syndrome,NS) 患儿外周血淋巴细胞(peripheral blood lymphocytes,PBLs)Bax蛋白的表达与PBLs凋亡的关系。探讨MsPGN-NS患儿糖皮质激素(glucocorticoid,GC)抵抗的部分机制。方法所有患儿和正常儿童均于清晨空腹条件下采取静脉血,以密度梯度离心法分离PBLs,所得细胞用植物血凝素(phytohemagglutinin,PHA)或PHA+1×10-6mol/L地塞米松(dexamethasone,Dex)处理,培养24 h和48 h后应用流式细胞仪检测Bax蛋白的表达及细胞凋亡率。结果①MsPGN-NS患儿与正常儿童Dex组PBLs Bax蛋白表达量均显著高于空白对照组(P0.01),且呈时间依赖性(48 h24 h),但各时间点MsPGN-NS患儿Dex组Bax 的表达均低于正常儿童相应组(P0.01);②MsPGN-NS患儿与正常儿童Dex组PBLs凋亡率均高于空白对照组(P0.01),且呈时间依赖性

(48 h24 h),但各时间点MsPGN-NS患儿Dex组细胞凋亡率均低于正常儿童组(P0.01)。结论MsPGN-NS患儿PBLs Bax蛋白表达水平与PBLs凋亡率呈明显正相关,但其表达显著低于正常对照组,提示Bax蛋白的低表达是导致MsPGN-NS患儿PBLs凋亡减少,进而造成GC抵抗的原因之一。

)Abstract:Ojective To investigate the relationship between expression of Bax protein and apoptosis in peripheral blood lymphocytes (PBLs) in children with primary nephrotic syndrome (NS) with the pathology type of mesangial proliferative glomerulonephritis (MsPGN), so to further explore the mechanism of glucocorticoid-resistance in children. Methods The PBLs in MsPGN-NS and normal children were isolated with Ficoll-Hypaque gradient centrifugation. Then they were treated with phytohemagglutinin (PHA) or PHA plus dexamethasone (Dex). At the 24th and 48th hour of culture, the expression levels of Bax protein and apoptosis proportion were measured by flow cytometric analysis. Results The Bax protein levels in PBLs of Dex in the control group were much higher than those in both the MsPGN-NS group and normal children (P0.01) in a time-dependence manner (48 h24 h). However, at each time point the Bax protein levels of

Dex group in MsPGN-NS children were all lower than those in normal children (P0.01). The apoptosis proportions of PBLs remarkably increased after Dex treatment in both MsPGN-NS and normal children (P0.01), also in a time-dependence manner. But at each time point the apoptosis proportions of Dex group in MsPGN-NS children were all lower than those in normal children (P0.01). Conclusion In PBLs of MsPGN-NS children the expression of Bax protein has positive correlation with apoptosis proportions, but at a level significantly lower than in the normal control group. The low expression of Bax protein probably accounts for the decreasing apoptosis proportions of PBLs, which leads to glucocorticoid-resistance in children with MsPGN-NS.

糖皮质激素(glucocorticoid,GC)是治疗肾病综合征(nephrotic syndrome,NS)的首选药物,但临床上部分NS患儿由于对GC抵抗,致使反复应用GC和免疫抑制剂,从而出现很多药物不良反应和(或)并发症,加重了原有的病理损害。因此NS患儿GC抵抗的原因一直是研究的热点[1-5]。系膜增生性肾小球肾炎(mesangial proliferative glomerulonephritis,MsPGN)是儿童原发性NS较常见的病理类型之一,也是最常见的对GC不敏感的病理类型,临床治疗较为困难。免
