对策，以降低处方差错发生率。方法选取该院西药房2012年1月—2016年1月期间发生的120例处方内差案例进行回顾性分析，对出现差错的原因进行调查分析，并总结相应改进对策。结果导致医院药房处方差错的因素较多，通过对该院西药房出现处方调剂差错的情况进行统计分析，发现主要存在品种差错、数量差错等方面的原因，其中药品的品种差错共有79例、数量差错共有36例，分别占比65.83%、30%，是调剂内差中最为主要的，应当引起足够重视。结论从该研究的120例处方差错分析情况，以及根据本人长期药房工作经验来看，医院药房出现处方调剂差错容易发生于药房工作的各个环节，笔者认为应当从规范药品陈列、严格遵守处方调剂操作规程、加强沟通与交流、加强医院药房管理、加强药房工作人员的管理培训等方面着手来降低该院西药房处方差错发生率。

Analysis of Common Reasons and Strategies of the Prescription Compounding Internal Error in the Hospital Pharmacy

[Abstract] Objective To analyze the common reasons of prescription internal error in the western medicine pharmacy，put forwards the targeted solutions in order to reduce the incidence rate of prescription errors. Methods 120 cases with prescription internal error in the western medicine pharmacy in our hospital from January 2012 to January 2016 were retrospectively analyzed，and the error causes were investigated，and the corresponding improvement strategies were summarized. Results There were many causes of prescription internal error in the hospital pharmacy，and the statistical analysis showed that the errors of variety and quantity were main reasons，including 79 cases with variety errors （65.83%）and 36 cases with quantity error （30%），which should arouse enough attention. Conclusion The analysis results and the long-term working experience of the writer show that the compounding error in the hospital pharmacy is easy to occur in various links in the pharmacy work，and the writer supposes that we should reduce the incidence rate of western medicine prescription error by standardizing the drug display，strictly follow the prescription compounding operation regulations，enhancing communication，hospital pharmacy management and management training of pharmacy working staff.
