Email: hejie@.Objective: The National Central Cancer Registry (NCCR) collected population-based cancer registrationdata in 2011 from all cancer registries. National cancer incidence and mortality were compiled and cancerincident new cases and cancer deaths were estimated. Methods: In 2014, there were 234 cancer registries submitted cancer incidence and deaths occurred in2011. All datasets were checked and evaluated based on the criteria of data quality from NCCR. T otal 177registries’ data were qualified and compiled for cancer statistics in 2011. The pooled data were stratified byarea (urban/rural), gender, age group (0, 1-4, 5-9, 10-14…85+) and cancer type. Cancer incident cases anddeaths were estimated using age-specific rates and national population in 2011. All incidence and death ratesare age-standardized to the 2000 Chinese standard population and Segi’s population expressed per 100,000persons.Results: All 177 cancer registries (77 in urban and 100 in rural areas) covered 175,310,169 populations(98,341,507 in urban and 76,968,662 in rural areas). The morphology verified cases (MV%) accountingfor 70.14% and 2.44% of incident cases were identified through death certifications only (DCO%)with mortality to incidence ratio of 0.63. The estimates of new cancer incident cases and cancer deathswere 3,372,175 and 2,113,048 in 2011, respectively. The incidence rate was 250.28/100,000 (males277.77/100,000, females 221.37/100,000), and the age-standardized incidence rates by Chinese standardpopulation (ASIRC) and by world standard population (ASIRW) were 186.34/100,000 and 182.76/100,000with the cumulative incidence rate (0-74 years old) of 21.20%. The cancer incidence and ASIRC in urbanareas were 261.38/100,000 and 189.89/100,000 compared to 238.60/100,000 and 182.10/100,000 in ruralareas, respectively. The cancer mortality was 156.83/100,000 (194.88/100,000 in males and 116.81/100,000in females), the age-standardized mortality rates by Chinese standard population (ASMRC) and by worldstandard population (ASMRW) were 112.88/100,000 and 111.82/100,000, and the cumulative mortality rate(0-74 years old) was 12.69%. The cancer mortality and ASMRC were 154.37/100,000 and 108.20/100,000in urban areas, and 159.42/100,000 and 117.97/100,000 in rural areas, respectively. Cancers of lung, femalebreast, stomach, liver, colon and rectum, esophageal, cervix, uterus, prostate and ovary were the mostcommon cancers, accounting for about 75% of all cancer new cases. Lung cancer, liver cancer, stomachcancer, esophageal cancer, colorectal cancer, female breast cancer, pancreatic cancer, brain tumor, cervicalcancer and leukemia were the leading causes of cancer death, accounting for about 80% of all cancer deaths. The cancer incidence, mortality and spectrum showed difference between urban and rural areas, males andfemales. Conclusions: The coverage of cancer registration population had a greater increase than that in the lastyear. The data quality and representativeness are gradually improved. As the basic work of cancer preventionand control, cancer registry is playing an irreplaceable role. The disease burden of cancer is increasing, andthe health department has to take effective measures to contain the increased cancer burden in China. Keywords:Cancer registry; incidence; mortality; epidemiology; ChinaSubmitted Dec 15, 2014. It was the second leading cause of death, and the incidence and mortality keep increasing (1-3). In 2008, the national program of cancer registry was launched by the Ministry of Health of China. Since then, National Central Cancer Registry (NCCR) of China has been publishing cancer statistics annually based on registries’ data (4). In this article, we analyzed the cancer incidence and mortality in cancer registration areas in 2011 and estimated the numbers of new cancer cases and deaths nationally to provide an overview of current cancer statistics using data collected from local registries. The updated cancer burden results would be broadly used by government, researchers and clinicians for policy and research.Materials and methods Data sourceThe NCCR is responsible for cancer data collection, evaluation and publication from local population-based cancer registries. The cancer information was reported to the cancer registries from local hospitals, and community health centers, including the Basic Medical Insurances for Urban Residents and the New-Rural Cooperative Medical System. The Vital Statistical Database was linked with the cancer incidence database for identifying cases with death certificate only (DCO) and follow-up. By 1 June 2014, 234 cancer registries (98 cities and 136 counties) from 31 provinces submitted 2011 data to the NCCR. Data covered about 221,390,275 people, accounting for about 16.43% of the national population. All cancer cases were classified according to the International Classification of Diseases for Oncology, 3rd edition (ICD-O-3) and the International Statistical Classification of Diseases and Related Health Problems 10th Revision (ICD-10). Invasive cases of lung cancer (ICD10: C33-C34) were extracted and analyzed from the overall cancer database. Incidence and mortality [2011] were based on data from 177 population-based cancer registries which distributed in 28 provinces (77 in urban and 100 in rural areas) and covered a total of 175,310,169 populations (98,341,507 in urban and 76,968,662 in rural areas) including 88,655,668 males and 86,654,501 females, accounting for 13.01% of the national population (Figure 1).National population estimatesThe national population in 2011 was estimated based on the 5th National Population Census data [2000] provided bythe National Statistics Bureau of China, taking into account of the changes of age composition, gender ratio and the proportion of urban and rural transformation released by the National Bureau of Statistics (/). The national population in 2011 was stratified by area (urban/rural), gender (male/female) and age groups (0-, 1-4, 5-84 by 5 years, 85+ years). The changes of age-specific death probability were also adjusted when calculating population. Linear changes were assumed in each age group between the fifth and sixth Population Censuses.Quality controlSubmitted data were checked and evaluated by NCCR based on “Guideline for Chinese Cancer Registration” and referring to relevant data quality criterion of “Cancer Incidence in Fi ve Continents Volume IX” by IARC/IACR (5). Software such as MS-FoxPro, MS-Excel and ICRC/IACR tools IARC-crgT ools was used for data collection, sorting, checking and evaluation. Proportion of morphological verification (MV%), percentage of cancer cases identified with death certification only (DCO%), mortality to incidence ratio (M/I), percentage of uncertified cancer (UB%) and percentage of cancer with undefined or unknown primary site (secondary) (O&U%) were used to evaluate the completeness, validity and reliability of cancer statistics.Statistical analysisIncidence and mortality rates were calculated byarea,Figure 1 Map of the 177 contributing cancer registries and geographic regions in China (the dots are location of the cancer registries).Chen et al. 2011 annual report of cancer in China© Chinese Journal of Cancer Research. The numbers of new cases and deaths were estimated using the 5-year age-specific cancer incidence/mortality rates and the corresponding populations. The Chinese population in 2000 and World Segi’s population were used for age-standardized rates. The cumulative risk of developing or dying from cancer before 75 years of age (in the absence of competing causes of death) was calculated and presented as a percentage. Software including MS-Excel, IARCcrgT ools2.05 issued by IARC and IACR was used for data checking and evaluation (6). SAS software (SAS Institute Inc., Cary, USA) was used to calculate the incidence and mortality rates. Results Data qualityThere were 177 registries accepted by this annual report distributed in 28 provinces, including 77 cities and 100 counties covering 175,310,169 of populations, including 88,655,668 males and 86,654,501 females, accounting for 13.01%.The overall indicators of MV%, DCO%, and M/I ratio were 70.14%, 2.44% and 0.63, respectively. They were 72.92%, 2.17% and 0.61 in urban registries, compared to 65.34%, 2.90% and 0.67 in rural areas. The quality evaluation for major cancers is presented in T able 1.Chinese Journal of Cancer Research, Vol 27, No 1 February 2015 © Chinese Journal of Cancer Research. All rights reserved.Chin J Cancer Res 2015;27(1):2-12 5Incidence and mortality of overall cancersIncidenceIt was estimated there were 3,372,175 new cases diagnosed as cancer in 2011. The crude incidence rate of all cancers was 250.28/100,000 (277.77/100,000 in males and 221.37/100,000 in females). The age-standardized incidence rates were 186.34/100,000 and 181.49/100,000 by Chinese population (ASIRC) and World population (ASIRW), respectively. Among the patients aged 0-74 years, the cumulative incidence rate was 21.28%. The crude cancer incidences rate and age-standardized rates in urban areas were higher than that of rural areas. The crude incidences in males and females were higher in urban than in rural. However, after adjusted by age, cancer incidences for males in urban and rural areas were conversed. More new cancer cases occurred in eastern areas than middle areas and western areas, however, crude incidence rates in three areas were not great different (T able 2).Age-specific incidence rateCancer incidence was relatively lower before 40 years old, then increased dramatically after 40 years old and finally peaked after 80 years and then slightly decreased after 85 years (T able 3, Figure 2). The pattern was similar between urban and rural areas. Comparing the age-specific incidence rate between urban and rural areas, except age from 55-64, urban had higher incidence rates in most of age groups. The incidence at 80 years old was higher in urban areas than in rural areas, but lower in the age group of 40-74 years. Before age of 70 years, most age groups of males in rural areas had higher incidence rate than that in urban. However, in females, the incidences were higher in urban than in rural areas in every age group after 25 years (T able 3, Figure 2).MortalityIt was estimated there were 2,113,048 died from cancer in 2011. The crude mortality of all cancers in China was 156.83/100,000 (194.88/100,000 in males and 116.81/100,000 in females). The age-standardized mortalities were 112.88/100,000 and 111.82/100,000 by Chinese population (ASMRC) and World population (ASMRW), respectively. Among the patients aged 0-74 years, the cumulative incidence rate was 12.69%. The crude cancer mortality rate and age-standardized rates in urban areas was higher than that in rural areas. The crude cancer mortalities were higher in urban than in rural, however, after adjusted by age, the mortalities were conversed both in male and female. More cancer deaths occurred in eastern areas than middle areas and western areas, however, western areas had higher mortalities than eastern and middle areas (T able 4).Age-specific mortalityThe cancer mortality was relatively lower before 45 years and then dramatically increased reaching peak after 85 years (T able 5, Figure 3). The mortality in rural areas was highest in the age group of 80-84 years. The age-specific mortality in urban males was lowered than that in rural in most of age-groups before 80 years old. In females, the mortality in urban was higher than that in rural only for age group over 75 years.Incidence and mortality for major cancersCancer incidence for the 10 most common cancersLung cancer was the most common cancer in all areas, followed by female breast cancer, stomach cancer, liverChen et al. 2011 annual report of cancer in China© Chinese Journal of Cancer Research. Other cancer types with high mortality in males were liver cancer, stomach cancer, esophageal cancer and colorectal cancer. In females, stomach cancer was the second cause of cancer death, followed by colorectal cancer, liver cancer and breast cancer (T able 9).Chen et al. 2011 annual report of cancer in China© Chinese Journal of Cancer Research. All rights reserved.Chin J Cancer Res 2015;27(1):2-12 8Cancer incidence of the 10 most common cancers in rural areasT able 10 shows the 10 most common cancer incidence rates in rural areas. Lung cancer has been becoming the most frequently diagnosed cancers (309,510 new cases with incidence rate of 47.14/100,000), followed by stomach cancer, esophagus cancer, liver cancer and female breast cancer. The most common sites of cancer in males were lung, stomach, liver, esophagus, and colon-rectum, while in females, they were cancers of lung, breast, stomach, esophagus and liver (T able 10).Cancer death of top 10 cancers in rural areasLung cancer was the leading cause of cancer death in rural areas for both males and females. The number of deaths on lung cancer in rural was 248,152 with mortality of 37.80/100,000. The other cancer types with high mortality were liver cancer, stomach cancer, esophageal cancer and colorectal cancer in males; stomach cancer, liver cancer, esophageal cancer and breast cancer in females (T able 11).DiscussionPopulation-based cancer registry in China collects the information on cancer incident cases and cancer deaths from covered population to surveillance the cancer epidemics in certain areas. The cancer registration data are not only applied for cancer control planning, implementation and evaluation on cancer prevention and control, but also for scientific research (7).In mid of 2014, the NCCR called for data submission in 2011 from all registries supported by national program of cancer registry. T otal 234 registries reported registration data including national registries and some provincialregistries. The population coverage of all registries was over 221 million, accounting for 16.4% of national populations. Stringent quality control is essential based on the national criteria issued in program protocol to ensure the data reliability. The incidence, mortality and population from a same registry have to keep reasonable levels compared to past data. The indicators of completeness, comparability and invalidity, such as MV%, DCO%, M/I ratio, UB% and O&U%, were evaluated for every registry’s data to judge if submitted data is valid or not. If any logical errors, inexplicable incidence, or mortality, the data would be sent back to registries to check and correct. Through the double evaluations in provincial and national level, 177 registries were qualified and accepted for final pooled database, which was used for cancer statistics in 2011. The population was estimated based on the fifth National Population Census data in 2000 released by the National Bureau of Statistics, and the changes of age construction, gender ratio, the extent of urbanization level and age specific death probability were taken into account (/). The national population and registries’ data in 2011 stratified by area (urban/rural), gender (male/female) and age groups were used for estimates of national cancer new cases and cancer deaths. In the annual report of 2010, the national population from the sixth Population Census was used for estimation, which residential population was registered (4). However, the household population is considered to be more suitable because the cancer data are based on the household registration. It was estimated that cancer incident new cases were 3.37 million and 2.11 million died in cancer all over the country in 2011 with the incidence rate of 250.28/100,000 and mortality of 156.83/100,000. Compared to the figures in 2010, there were 280 thousands and 150 thousands more on cancer new cases and deaths with 6.4% and 5.4% increases of incidence rate and mortality due to the different population mentioned before. After adjusted by age, the incidence and mortality were kept stable. Therefore, the cancer statistics in 2011 were reasonable and creditable. There are still few registries in the west due to the low population density. The western region would be the emphasis for building new cancer registries to meet the requirement of the national and regional representativeness of cancer registry (8).Urban areas in China had a higher cancer incidence and lower mortality than those in rural areas and the cancer patterns are quite different (2,3). Recent study showed the differences narrowed (9,10). The reason might be high prevalence of tobacco smoking (11) and the air pollution due to the rapid development of urbanization and industrialization (12). A population-based cancer survival study displayed that rural China had a much low relative 5-year survival rate compared with urban China (13,14). This could be explained by limited medical resources, low level of cancer cares and more patients with late stage of cancers. The government should notice the differences between urban and rural areas and implement effective measurement to bridge the gap.Cancers from upper digestive organ are still the more common cancers and cancer deaths in China, and the incidences of lung cancer, breast cancer and colorectal cancer are increasing. Thyroid cancer had ranked the eighth frequent cancer in China and fifth in urban females that needs to be noticed. The improvement of diagnosis might be the key reason, but the further etiological study is necessary. In 2014, the number of registries has been increased to 308, covering 300 millions of population. An extra budget was settled to support follow-up and survival analysis from 2014. The registration data would be getting more valuable and play an important role on cancer control. The National Cancer Center launched a project of cancer atlas, which would provide useful information about geographic distribution of cancer to make national cancer control program more efficient.AcknowledgementsWe gratefully acknowledged the cooperation of all the population-based cancer registries in providing cancer statistics, data collection, sorting, verification and database creation. Overuse of colorectal cancer screening services in the United States and its implications

Overuse of colorectal cancer screening services in the United States and its implications

Bian C hin J Cancer (2016) 35:88 DOI 10.1186/s40880-016-0148-5Overuse of colorectal cancer screening services in the United States and its implicationsJohn Bian *AbstractAs a standard way for prevention and early detection of colorectal cancer (CRC), colonoscopy has been used for CRC screening in the United States for more than one decade. An article entitled “Assessing Colorectal Cancer Screening Adherence of Medicare Fee-For-Service Beneficiaries Age 76 to 95 Years” recently published at the Journal of Oncology Practice reports the trends in overuse of CRC screening services among average-risk elderly populations at the age of 76–95 years. Several reasons for overusing colonoscopy have been postulated, and some strategies for reducing overuse of CRC screening services have also been proposed. Keywords: Colorectal cancer, Screening services, Colonoscopy© 2016 The Author(s). She appeared stressed out and very tired. I was concerned and inquired about how had been with her. She told me that she had been looking after her sick mother in the past 4 months. Her mother, in her mid-eighties, had been very healthy until she was hospitalized for pneumonia last December in 2015. During her hospitalization, her doctor seemingly randomly asked whether she ever had colorectal cancer (CRC) screening tests. After told no test being done in the past, her doctor ordered a screen-ing colonoscopy while she was in the hospital. Unfor-tunately, this procedure led to perforation and forced her into a prolonged hospital stay. Her mother was still recovering from this colonoscopy-related complication. An article entitled “Assessing Colorectal Cancer Screen-ing Adherence of Medicare Fee-For-Service Beneficiaries Age 76 to 95 Years” recently published at the Journal of Oncology Practice (JOP ) by Bian et al. [1] highlighted the trends in the overuse of CRC screening services amongaverage-risk elderly populations at the age of 76–95 years from 2002 to 2010. CRC screening is an effective and a cost-effective way for reducing CRC-related death. However, these screening services, particularly colonos-copy, may carry some risks. Specifically, screening colo-noscopies may expose patients to polypectomy-related perforation and bleeding risks and to sedation-related cardiovascular and pulmonary complications. The level of these risks may also be elevated with age. As a result, after weighing in the benefit-and-risk tradeoff, the United States (US) Preventive Services Task Force (USPSTF) does not recommend routine CRC screening to indi-viduals over age 75 years at an average risk of CRC (e.g., without family history of CRC or prior polyp/adenoma history) [2]. In spite of the above explicit recommendation by the USPSTF, almost all major health insurance pro-grams in the US, such as the Medicare program (a nation-wide health insurance for the elderly age over 65 years), have continued their coverage for CRC screening of the average-risk individuals without setting upper-age limit. Consequently, there has been a growing concern about potential overuse of CRC screening services after Medi-care started coverage of all four main CRC screening modalities (including colonoscopy) in 2001 [3].Chinese Journal of Cancer*Correspondence: jbian@Department of Clinical Pharmacy and Outcomes Sciences, South Carolina College of Pharmacy, Columbia, SC 29208, USA 495Bian C hin J Cancer (2016) 35:88This JOP article [1] reports an observational study designed for addressing this concern. This study used a 5% random non-cancer sample of Medicare fee-for-ser-vice (FFS) enrollees during 2002–2010 residing in the Surveillance, Epidemiology, and End Results (SEER) areas to construct a 9-year study cohort that included average-risk enrollees at the age of 76–95 years. The two out-comes of interest were the up-to-date status (in a given year) of adherence to overall CRC screening and to colo-noscopy (versus the other three modalities). The authors additionally analyzed a sample of average-risk enrollees at the age of 86–95 years, in which any screening services received may be deemed overuse of CRC screening ser-vices according to the USPSTF recommendation. The authors found that overall CRC screening adherence rates for studied individuals at the age of 76–95 years rose from 13.0% in 2002 to 21.4% in 2010. In 2002, 2.2% were adherent to colonoscopy, and 10.7% to the other three modalities; in 2010, the corresponding rates were 19.5 and 1.9%. The rapid rise in overall adher-ence rates may be a result of Medicare coverage policy of screening colonoscopy for average-risk beneficiaries starting in 2001, and a longer recommended screening interval of 10 years. Our additional analysis of the enroll-ees at the age of 86–95 years demonstrated that the over-all adherence rates were almost doubled from 6.7% in 2002 to 12.5% in 2010. These increases, largely driven by colonoscopy use, may represent a significant portion of overuse of CRC screening services. There are at least three plausible reasons for the observed potential overuse. First, the direct financial incentive in a Medicare FFS environment may promote over-prescription of these services. Second, because the CRC screening adherence rate has been widely used as a quality indicator for measuring performance of health-care provider, perceived higher rates may suggest higher health care quality and in turn may indirectly lead to higher payment for better performance. Third, because many providers may be overwhelmed by day-to-day busy clinical practice, particularly in primary care settings, they have little time for consulting patients about the benefits and risks of various cancer preventive services such as CRC screening. In the end, the authors cautiously offered some corresponding remedies for reducing over-use of CRC screening services: redesigning value-based payment systems, measuring age-weighted CRC screen-ing performance, and reimbursing physicians for their time discussing the trade-off of benefits and risks of screening colonoscopy. Finally, this JOP article [1] may offer some insights of using observational data for cancer health services/out-comes research in China. The dataset used in this study was longitudinal non-cancer data, paralleled to the SEER data (the cancer registry data covering a quarter of the US populations). A large body of evidence has been pub-lished from these datasets on cancer topics related to epidemiology, prevention, treatment, and survivorship. In addition, these datasets have been increasingly used for cancer health policy research [4, 5]. Undoubtedly, the evidence generated from these valuable datasets may have contributed to remarkable improvement in cancer survivorship in the US over the past four decades. As cancer death is becoming a major health burden in China [6, 7], more research that uses nationally representative cancer registry and other observational data is critically needed to monitor progresses of quality and outcomes of cancer care in China.Received: 10 June 2016 Accepted: 11 June 2016References1. Cervical cancer is the second most common malignancy in women, which affects women's health severely.According to WHO database, there are about500000cases of newly diagnosed cervical cancer and274000cases of cervical cancerrelated death in every year,83%of the cases are from developing countries, accounting for15%of women's malignancies;while in developed countries, cervical cancer only accounts for 3.6%of women's malignancies. 1 The risk factors, newly diagnosed cases, morbidity and mortality of cervical cancer in developing counties are all on the rise, which brings great anguish to the patients and their families.Cancercaused reduction of labor force will negatively impact the social development. If prompt strategies are not initiated, the mortality of cervical cancer patients will increase by25%in the following ten years. 2 How to prevent and control cancers has become a global healthissue.The2008Nobel prize of physiology and medicine is sharedby a German scientist Harald zur Hausen and two Frenchscientists Fran觭oise Barré Sinoussi and Luc Montagnier.Theachievement of Dr.Hausen was the discovery that humanpapillomavirus(HPV)was the cause of cervical cancer.Cervicalcancer has become one of a few cancers whose etiology hasbeen defined. This discovery elucidated the pathogenesis ofcervical cancer and natural history of HPV infection, providedtheoretical basis of the causal relationship between HPV infectionand cervical cancer, thereby, paved the way for the quickdiagnosis of cervical cancer and its precancerous lesions by HPVDNA detection and the development of the first cancer vaccine要cervical cancer prophylactic vaccine in human history.Hence, cervical cancer will be the first malignant tumor to be preventedor even eradicated through comprehensive strategies ofvaccination, screening, early diagnosis and treatment. 3Up to now, more than200strains of HPV have beenidentified,of which,15strains can cause cervical intraepithelialneoplasia(CIN)and cancer.CIN usually occurs several yearsearly prior to cervical cancer, which lends rationale that earlyscreening and effective intervention can avoid the occurrence ofcervical cancer.Pap smear has been widely used as a cervicalcancer screening test for over half a century, and it has madegreat contribution to the prevention of cervical cancer. With theprogress of cytological techniques, liquidbased cytology(LBC)and computerassisted diagnosis with an automatic slidesprocess system have been approved for the use of cervicalcancer screening in the mid1990s.The technique has not onlyimproved the quality of slides and accelerated slides interpreting, but also promoted the sensitivity and specificity of pathologicidentification to87%and94%,respectively,reduced the falsenegative rate greatly. The following HPV DNA detection Perspective of cervical cancer prevention and control in developing countries and areasYouLin QiaoDepartment of Epidemiology,Peking Union Medical College, Chinese Academy of Medical Sciences,Beijing100021,P.R. generation hybrid capture (hc2),have improved the sensitivity and specificity of cervical cancer and precancerous lesion detection to95%and85%, respectively. This is a breakthrough in the field of cervical cancer screening, greatly improving the quality of cervical cancer screening. 4 In2005,WHO announced that there were sufficient evidence showing that HPV DNA detection can be used for cervical cancer screening and can decrease the mortality and morbidity of cervical cancer. 5 An Indian prospective study lasted for eight years showed that,as compared with controls,HPV DNA(hc2)screening was able to significantly reduce the mortality and morbidity of advanced cervical cancer(by50%). 6 Recently,a HPV rapid screening technique(CareHPV)that was sponsored by Bill&Melinda Gates Foundation has been tested in China, which provides a convenient, simple and economical method for the early detection and intervention of cervical cancers in developing regions. 7 The ever progressing techniques for cervical cancer screening, especially the development of HPV rapid screening techniques, open a new era of the cervical cancer prevention andcontrol. The etiology of cervical cancer is clear, the techniques forearly detection and prevention are mature, and multiple diagnosisand treatment methods are available in regions of variouseconomical levels. Thus, the prospect of cervical cancer controlis promising.WHO recommended screening, early diagnosis andintervention of cervical cancer in worldwide, and considered thatcervical cancer prevention is less dependent on the abundance ofresources, but on the government's attitude and effectiveness ofhealth organizations. 2 Currently, the medical insurances of mostcountries cover cervical cancer screening, which greatly reducesthe mortality and morbidity of cervical cancer.Epidemiological data of developed countries and somedeveloping countries with screening programs show thatorganized cervical cancer screening program(primarily based oncytology and HPV detection)can significantly reduce the mortalityand morbidity, which has been reached a consensus. 2Internationally,advanced cervical cancer and associated deathare regarded as ineffectiveness indices of medical resourceavailability and health care equality. 2 Therefore, theimplementation of health care measures to prevent cervicalcancer reflects the priority of women(especially middle agedwomen)in a society,as well as the civilization and progress of acountry. However, this measure has not been employed bydeveloping countries, and the goal to screen cervical cancer formiddle aged women has not been realized. This is probablyattributed to the imbalance of health care resources in developingcountries, limited medical resources are used in a smallpopulation.Secondly, the lack of quality control system of cytologicalscreening and qualified cytotechnologist and cytologists arecommon problems in developing countries and regions.Papsmear is unfavorable due to its low sensitivity(high false negativerate),and because the screening result is not immediatelyavailable, many women who underwent screening are lost tofollowup, therefore, this technique cannot be widely employed indeveloping countries. 8 Using Pap smear to extensively andrepeatedly(every two to three years)screen cervical cancerfaces numerous difficulties and challenges.Due to theseshortcomings of Pap smear,it is urgent to establish a cervicalcancer screening technique which is simple, rapid, effective andeconomical and is suitable to be used in developing countries. In recent years,WHO/IARC conducted a populationbased, randomized, controlled, largesample trial in India, and provedthat with the prerequisites of shortterm training and effectivequality control, visual inspection with acetic acid/iodine solution(VIA/VILI)can be used as an effective cervical cancer screeningmethod in developing countries. 9 It does not require laboratoryplatform,is technically undemanding, easy to master and cheap, which can be performed by experienced physicians or othermedical workers(such as nurses and midwives). It is the primarycervical cancer screening method recommended by WHO to beused in developing countries.HPV rapid screening test(careHPV)which is suitable for developing countries has recentlybeen developed and is now under registration trial. It is expectedto be put into market in2010and will be promoted byWHO.Using safe and effective HPV vaccine is an importantmeasure to block HPV transmission and prevent cervical cancer. The development of cervical cancer vaccine is no doubt themilestone in the history of combating cancer, and is a big stepforward in the research of human cancers.Since2007,in allcancer control meetings organized by WHO,member states havebeing discussing the prospect of cervical cancer control with theutilization of HPV vaccine. All members agreed that,in an effortto reduce cervical cancer burden, every county should enhancethe cervical cancer screening program, improve strategies oftreatment and monitoring, and introduce HPV vaccine inappropriate regions. In November2008,the results of all regionalmeetings and a comprehensive introduction of HPV vaccinebackground were hand over to Strategic Advisory Group ofExperts(SAGE)on Immunization, proposing the global HPVvaccine immunization policy to share this stateoftheartachievement of cervical cancer vaccine with developingcountries. 10The undergoing medical reform in China makes a greatopportunity for this intervention.Experts have been discussing thespecific steps to carry out cervical cancer screening program. The Ministry of Health and Cancer Foundation of Chinaestablished the demonstration base of cervical cancer earlydetection and treatment in2005,the Ministry of Finance fundedthe cancer early detection and treatment programs in2006,andtenmillion rural woman cervical cancer screening program isinitiated in2009.The cervical cancer prevention has drawntremendous attention from the government and public in China.Although the choice of specific method for cervical cancer 22010;Vol.29Issue1 咱员暂 咱圆 暂 咱猿 暂 咱源 暂 咱缘 暂 咱远 暂 咱苑 暂 咱愿 暂 咱怨 暂 咱员 园 暂 screening is debated,we are glad to see many administrations that concerning cancer, reproductive health, adolescent health, immunization and gender equity are eager to take part in the cervical cancer prevention cooperatively. In making specific steps of cervical cancer prevention program,we have to face the challenges of local financial ability, feasibility of service ability and culture acceptability, which require the cooperation of administrations responsible for vaccine immunization, cancer prevention and reproductive health. There will be numerous unpredictable difficulties other than the price and techniques before the conduction of the program. Some difficulties are originated from attitude and awareness, thus,we have to eradicate the prejudice towards the simple screening technique that can be mastered by local medical workers.Currently, the most important thing is to promote the awareness of health requirements, costeffectiveness and the prospects of upcoming novel technique, thereby the leaders and administrators can be prepared to face the coming opportunities and challenges.Cancer prevention program should be integrated into a comprehensive health care service system and managed as a whole. We should spare no effort to extend the cervical cancer prevention strategies of cervical cancer vaccine inoculation, precancerous lesion screening and treatment to Chinese women. As a result, the measures fitting for women of appropriate ages and aiming to prevent cervical cancer radically will be issued by the government in the near future to eradicate the threats imposed by cervical cancer.Parkin DM, Bray F, Ferlay J, et al. Cell cycle.independent roles of p19INK4d in human terminal erythropoiesis

Han and Liu C hin J Cancer (2017) 36:22 DOI 10.1186/s40880-017-0189-4Cell cycle-independent roles of p19INK4d in human terminal erythropoiesisXu Han and Jing Liu *© The Author(s) 2017. The Creative Commons Public Domain Dedication waiver (/publicdomain/zero/1.0/) applies to the data made available in this article, unless otherwise stated.Normally, cyclin interacts with cyclin-dependent kinase (CDK) to form a cyclin-CDK complex, which promotes cell cycle progression, whereas cyclin-dependent kinase inhibitor (CDKI) molecules inhibit the formation of cyc-lin-CDK complex, arresting cell cycle. Terminal erythro-poiesis is closely coordinated with cell cycle exit, which is regulated by cyclins, CDKs, and CDKIs [1]. In the global transcriptome of human terminal erythropoiesis [2], p19INK4d is expressed highly, and its expression is signifi-cantly up-regulated during human terminal erythropoie-sis. However, the roles of p19INK4d in terminal erythro-poiesis are still unknown. As reported in our article recently published in Blood entitled “Unexpected roles for p19INK4d in posttranscrip-tional regulation of GATA1 and modulation of human terminal erythropoiesis” [3], we demonstrated what roles p19INK4d plays in human terminal erythropoiesis. We found that, in the erythropoietin-induced, CD34-positive hematopoietic stem cell (HSC) differentiation system, knockdown of p19INK4d delays terminal erythroid differ-entiation, inhibits erythroblast growth due to increased apoptosis, and leads to the generation of abnormally nucleated late-stage erythroblasts. Unexpectedly, knock-down of p19INK4d did not affect cell cycle, and these func-tions caused by p19INK4d knockdown were via decreasing levels of GATA-binding protein 1 (GATA1). Furthermore, we found that p19INK4d modulates GATA1 protein levels through a novel pathway, the phosphatidylethanolamine-binding protein 1 (PEBP1)-phosphorylated extracellular signal-regulated kinase (p ERK)-heat shock 70 kDa pro-tein (HSP70)-GATA1 pathway [3].As a classical CDKI member, p19INK4d has been shown to inhibit the formation of cyclin D-CDK4/6 complex, arresting cell cycle in the G 0/G 1 phase [4]. p19INK4d was often induced to inhibit the proliferation of many kindsof tumor cells, such as T cell acute lymphoblast leuke-mia cells and lung cancer H1299 cells [4, 5]. Additionally, deletion of p19INK4d leads to the development of many tumors, including osteosarcomas [6] and anterior lobe tumors [7]. p19INK4d is also involved in HSC quiescence, megakaryocyte differentiation, and granulocytic differen-tiation, which are closely associated with cell cycle arrest [8–10]. However, as shown in our study, p19INK4d played important roles independent of cell cycle regulation, and the lack of cell cycle change was probably due to the com-pensatory up-regulation of p18INK4c following p19INK4d knockdown. Our findings provide greater understanding of the role that CDKIs play in cell cycle regulation. In conclusion, our study revealed the cell cycle-inde-pendent roles of p19INK4d in human terminal eryth-ropoiesis via a novel PEBP1-p ERK-HSP70-GATA1 pathway. These findings will likely improve understand-ing of disordered erythropoiesis, including thalassemia, myelodysplastic syndrome, and congenital dyserythro-poietic anemia, and guide future studies that focus on CDKIs.Authors’ contributionsXH wrote the manuscript.
