【摘要】目的观察CO2气腹的不同腹内压(IAP)对肝细胞凋亡的影响.方法 24只
雄性健康家兔,体重3.0~3.5kg,根据CO2气腹的IAP随机分为4组:0mmHg组(C 组)、10mmHg组(Ⅰ组)、15mmHg组(Ⅱ组)和20mmHg组(Ⅲ组),每组6只.每
只家兔均接受相应IAP的气腹1h.气腹后12h测定肝功能,并通过western blot检测肝组织Bax和Bcl-2的表达.结果与C组比较,Ⅰ组、Ⅱ组和Ⅲ组的总胆红素(TB)、清蛋白(A)无明显变化,Ⅰ组的谷丙转氨酶(ALT)和谷草转氨酶(AST)显著增加(P<
0.05),而H组和Ⅲ组增加更明显(P<0.01),Ⅱ组的前清蛋白(Pre-A)显著降低(P<
0.05),而Ⅲ组降低更显著(P<0.01).与C组比较,Ⅰ组、Ⅱ组和Ⅲ组的肝组织Bax蛋白表达显著增加(P<0.01),而Bcl-2的蛋白表达水平在几个组均未见显著差别.结论CO2气腹可导致明显的肝功能损害,IAP越高,损害程度越重,其机制可能是IAP升
高促进了肝细胞凋亡所致.%Objective To observe the effects of different
intra-abdominal pressures (IAP) on hepatocyte apoptosis during CO2 pneumoperitoneum in rabbits. Methods According to the IAP of C02 pneumoperitoneum, 24 healthy male rabbits of weig hing 3. 0 ~ 3. 5 kg were randomly divided into 4 groups: 0 mmHg group ( group C) , 10 mmHg group ( groupl ) , 15 mmHg group ( groupll) and 20 mmHg group ( group M ). After 12 h of CO 2 pneumoperitoneum, hepatic function was assayed with an automatic biochemical analyzer and expressions of
hepatic Bax and Bcl-2 were detected with western blot assay. Results No signifcant difference was found in the levels of total bilirubin (TB) and albumin (A) in all groups. The levels of alanine aminotransferase (ALT) and aspartate aminotransferase (AST) in groupl were higher than that in group C (P< 0. 05) , and more significant ele vations were found in groupll and group Ⅲ ( P < 0. 01). Serum concentration of pre-albumin ( Pre-A) was reduced in groupll compared with group C ( P < 0. 05), but in group ID , it was obviously lower than that in group C (P< 0.01). The expression of hepatic Bax was significantly increased in groupⅠ,Ⅱ and Ⅲ (P < 0.01) , but there was no difference in the expression of Bcl-2 between groupsⅠ ,Ⅱ and Ⅲ and group C. Conclusion CO2 pneumoperitoneum can result in hepatic injury; and the higher the IAP is, the severer the injury is. One of the mechanisms may be that hepatocyte apoptosis is promoted by elevating IAP in rabbits.
