兹委托你单位对下列产品按下述要求进行检验, 我们将就此项委托给予积极配合及提供必要条件，并及时向你单位支付有关费用。

此外, 关于下述产品的图纸/技术资料，我单位特声明在其使用及提交上系完全合法。

This is to entrust China Classification Society (CCS) to survey the following products according to the following requirements. The Client agrees to provide an active cooperation and necessary for CCS in the course of survey and pay the relevant fee and expenses for the specified service. Furthermore, the Client guarantees that all contents declared in this certificate of entrustment and all materials submitted are true, and the use and supply of the technical documents of the following products are legally justified.

申请方已了解：本申请一经申请方签署和盖章并被中国船级社受理，即构成申请方与中国船级社之间的合同证明。

申请方保证在本申请表中所填写的内容和所提供的图纸、资料均属实，并且完全合法拥有提交的图纸资料。

如查有不实之处，申请方应负法律责任，承担由此造成的一切后果。

申请方保证提供认可条件，并按规定支付认可费和验船师为执行认可工作所必需的差旅费和其它开支。

即使此项认可由于申请方的原因未能由船级社完成，申请方也同意向中国船级社支付上述相关费用。

The Applicant acknowledges: Once this Application with the Applicant’s signature and stamp is processed by China Classificati on Society, This Application shall be deemed the evidence of contract between the Applicant and CCS. The Applicant guarantees that all contents declared in this Application and all materials submitted are true, and the use and supply of the following technical documents of the products are legally justified. If any false data are found, the Applicant shall assume legal responsibility, and hold responsibility for all the serious consequences arising therefore. The Applicant agrees to provide an active cooperation and necessary for CCS in the course of approval and pay the fee for the specified service and, in addition, any traveling and other expenses incurred by the surveyor related to the services. In the event of this project not being completed under the Society's services because of the Applicant’s reason, the Applicant also agrees to pay the relevant fee and expenses for the services provided.
