监测糖化血红蛋白在妊娠期糖尿病诊断中的临床价值

【摘要】目的：探索糖化血红蛋白监测对妊娠期糖尿病诊断的临床意义。

方法选
择2011年1月至2013年5月在我院治疗的妊娠期糖尿病患者100例和正常妊
娠孕妇100例，按照孕妇是否患有糖尿病分为研究组和对照组。

研究组患者以及
对照组患者都进行了常规的空腹血糖的测定，还口服葡萄糖50 g进行筛查测验，加上糖化血红蛋白的测定，比较两组测定结果。

结果研究组的FPG测定、GCT测定和HbA1c测定平均值都明显大于对照组的对应测定值。

研究组的FPG阳性率、GCT阳性率和HbA1c阳性率分别为38%、75%、71%，也明显高于对照组的7%、25%、5%。

HbAlc测定方法的灵敏度、可靠性、特异性和阳性预测效率达
到了83.5%、95.5%、90.5%、95.0%，远远高于其他两种测定方式的效率。

结论HbA1c测定方法具有良好的灵敏度、可靠性和特异性，具有较高的阳性预测效率，临床应用价值高。

%Objective To explore the clinical significance of glycosylated hemoglobin monitoring of gestational diabetes
diagnosis.Methods From January 2011 to May 2013 in gestational diabetes in our hospital 100 cases and 100 cases of normal pregnancy, according to whether the pregnant women with diabetes are divided into study group and the control group. For patients and control patients were determined research groups are routine fasting blood glucose, also 50g oral glucose screening test, with glycated hemoglobin measurement results were
compared.Results FPG study group determination, GCT measuring HbA1c and the mean measurement values measured are significantly higher than the corresponding control group. FPG positive rate of the study group, GCT positive rate and HbA1c positive rates were 38%, 75%, 71%, also significantly higher than 7%, 25%, 5%. Determination of the sensitivity of HbA1c, reliability, specificity, and positive predictive efficiency reached 83.5%, 95.5%, 90.5%, 95.0%, far higher than the efficiency of the other two measurement methods.Conclusion HbA1c measurement method has good sensitivity, reliability and specificity, with a high positive predictive efficiency, high clinical value.
